Introduction with Problem Statement In today’s complex and high pace health systems, the power of the individual is entirely ineffective compared to that of a highly efficient and collaborative team. However, these professional healthcare organization systems can be tarnished by the presence of “tribes” among nurses, physicians, students, and etcetera. “Tribal culture” is literally a metaphor for groups that emphasize the exclusion of outside members into their “cliques.” In the movie “The Faces of Medical Error…from tears to transparency – The Story of Lewis Blackman”, there was a lack of communication and no transparency between different departments of the hospital. In Blackman’s case, the tribal culture led to the nurses failing to communicate vital health information to the physicians and patients, which eventually led to Blackmans death. So whether there are cliques of nurses, physicians, or even sub groups within each, the presence of them suppresses teamwork and collaboration. Background Lewis Blackman was a fifteen year old boy whose death was triggered by the numerous errors caused by his healthcare professionals. He was suffering from a Pectus Excavatum, a non-life threatening condition. His mother, Helen Haskell, was attracted to an advertised surgery that boasted all the benefits of the procedure without proclaiming the risks. Furthermore, there should be enough trust between the nurses and physicians where they can easily put aside their egos and ask for a second opinion when they have any doubts concerning a patient's safety. This was clearly exemplified when the nursing staff attending to Lewis Blackman failed to contact the physician when various side effects arose; instead they tailored the signs to fit the expected side effects. Even after Blackman’s health was deteriorating, the nurses remained in their “tribes” and never once broke out of it to ask for help. The entire hospital was built on strong culture of remaining in their tribes instead of having goals oriented towards patients care and safety.
Chasing Zero is a documentary which was meant to both educate the viewer on the prevalence of medical harm as well as to enlighten both the public and health care providers on the preventability of these events (Discovery, 2010). The documentary expounded on the fact each year more people die each year from a preventable medical error than die due to breast cancer, motor vehicle accidents or AIDS (Institute of Medicine, 1999). Medical harm can result from adverse drug events, surgical injuries, wrong-site surgery, suicides, restraint-related injuries, falls, burns, pressure ulcers and mistaken patient identities (Institute of Medicine, 1999). We are the liaison between the patient and the rest of the care team. We need to identify a potential problem or issue, and bring it to the attention of the whole care team. We should facilitate mutual trust, respect, shared decision-making, and open communication among all relevant persons in the care of the patients. With the collaborative support of the whole care team, the patients are in a much better position and are more likely to make an informed and deliberate decision. Although I respect and trust nurses and doctors, I always carefully observe what is being done with myself or my family members. After watching Josie’s story and being in the process of becoming a medical assistant, I feel this story has given me an initiative to ensure patients and their families are kept safe. The generation we live in is technological, there are many resources for patients and families to utilize to educate themselves when it comes to medical conditions. Some people like to self-diagnose and it makes it harder for doctors and healthcare workers to work with those patients. The health care workers in the show Nurse Jackie, are portrayed as as hard working professionals who hold a high regard for patient care. The nurses follow an established dress code which requires clean and preferably wrinkle free scrubs, as well as, a stethoscope. Since the nurses work in the ED they have had to develop strong relationships with the doctors to ensure that everyone can be trusted (Austin, 2009; Brixius, Wallem, & Dunsky, 2009; Hudis, 2009). For the most part the nurses know that the doctors hold the ultimate control, but with increasing trust the nurses can be empowered to exercise a broader scope of practice. In his book, Mountains Beyond Mountains, Tracy Kidder reports the journey of Dr. Paul Farmer in providing health care for everyone including those that cannot afford it. At one point, Farmer is not in Haiti and must entrust the care of a patient, John, to one of his workers, Serena Koenig. Farmer left important decisions up to Koenig despite not having as much experience; Koenig is able to obtain treatment for John in Boston where he passes away (Kidder 2003). Had Farmer not trusted Koenig enough to deal with John, it is possible that he would have passed away suffering to the very last moments of his life. In situations where a health care professional is away, he or she must entrust colleagues with the care of the patients. After the situation with John, Kidder confronts Farmer on whether or not Koenig had made the correct decision to spend resources on easing the death of a patient. Farmer responds that he has, “fought the long defeat and brought other people on to fight the long defeat” (p. 288). Farmer attempts to convince that fighting for patients no matter the cost is the essential belief of Partners in Health. In this case, Kidder began to mistrust that Farmer was making the most logical decisions, so Farmer clarified his beliefs in providing health care. The nursing profession is a profession where people put their trust in you to provide care that is not only effective, ethical, and moral, but safe. Not all health situations are simple or by the book. Not all hospitals have the same nurse-patient ratios, equipment, supplies, or support available, but all nurses have “the professional obligation to raise concerns regarding any patient assignment that puts patients or themselves at risk for harm” (ANA, 2009). When arriving at work for a shift, nurses must ensure that the assignment is safe for not only the patients, but also for themselves. There are times when this is not the situation. In these cases, the nurse has the right to invoke Safe Harbor, because according the ANA, nurses also “have the professional right to accept, reject or object in writing to any patient assignment that puts patient or themselves at serious risk for harm” (ANA, 2009).
