Fulfillment in healthcare, as with any endeavor, is about managing hopes. Probably the group best equipped to deal with the changes wracking the profession today is new providers, who are not so weighed down by great expectations. Providers need an internal compass to navigate the changing landscape of our profession. I believe this is the key to coping with the stresses of contemporary healthcare: identifying what is important to you, what you believe in, what you will fight for, and finding a supportive team. Residency programs can help in providing this support; instilling professionalism early on and assessing it frequently throughout training. Introducing students to virtuous mentors and creating time to foster that meaning can make all the difference. Just as a patient needs a navigator on a new health concern; a freshman provider needs navigation in a new practice. The medical field is among the largest and ever growing career fields, especially when dealing with Physician Assistants (PAs) and Nurse Practitioners (NPs). In the 1960’s when the physician shortage began, the medical field created the PA and NP positions to fill in the gaps (Curren, 2007, p. 404). This matter has opened up numerous questions as more and more PAs and NPs begin practicing, especially concerning their education level. Many patients are concerned that they will not get the proper care. PAs/NPs are beneficial to everyday life by providing patients with the necessary skills needed to successfully treat them.
My vision for the Family Nurse Practitioner in a primary healthcare setting is to bring health care to small communities, comprised of predominately poor working class people, who are classified as underprivileged. Performing exams on the sick, providing immunizations to the young, educating all, and assisting the elderly with chronic illness and disease management, while providing preventative care and education on wellness. Mentors rather than preceptors are investing in new grad nurses for longer than just a few months. NGNs are receiving constructive criticism from their mentors and serve as mediators between workplace issues. High levels of anxiety are reported in the first few weeks of employment, but with mentors there to have a caring attitude, the levels of anxiety can be greatly decreased and learning can be much easier facilitated (2012).
The NP core competencies are independent practice, ethics, health delivery system, policy, technology and information, practice inquiry, quality, leadership, and scientific foundation are accomplished by mentored patient experiences. However, stress on independent and interprofessional practice is essential (Thomas, Crabtree, Delaney, Dumas, Kleinpell, Logsdon, Marfell, & Nativio, 2012). Therefore, the NP and other providers must work in a collective fashion that includes mutual respect among all individual healthcare team members.
My personal advanced practice nurse philosophy is to provide the best care to my patients primarily by staying up to date on current practices. Staying up to date allows for the patient to receive care based on evidence. Through assessing, diagnosing, managing and treating patients I plan to take on all functions of a nurse practitioner to the best of my ability. Advocating and educating patients are two words that I will live by as a practitioner. As a future nurse practitioner, I have given immense thought to the selection of a clinical practice based on the primary care setting that utilizes the collaboration model. I have selected this type of clinical practice because it best suits my professional and personal goals. I value autonomy while having the ability to work within a healthcare team and enjoy teaching my patients. Nurse practitioners (NPs) are a valuable part of the healthcare team. In the 1960s, from a vision to improve primary healthcare to under-serviced communities, the development of the first NP program developed out of need as a public service and focused on the care of pediatrics and since then the care of other specific populations such as families, adults, geriatrics, and women health has emerged (Anderson & O’Grady, 2009, p. 380).
Conflict often arises between new graduate nurses and experienced staff over the ideal patient care. The realities of practice often turn out to be contrary to what graduates were taught in university (Phillips, Kenny, Esterman, & Smith, 2014). These discrepancies in practices have been shown to create distress and reality shock for new RNs (Gardiner & Sheen, 2016). Reality shock is experienced when the considerate effort is put into preparing for a specific employment and new employee finds the candidate not prepared at all (Harwood, 2011). Healthcare is viewed in an unrealistic way by most individuals. Many people view a physician as the only means to find a solution to their problem. Nurses are still seen by some as simply “the person who does what the doctor says.” This is frustrating in today’s time when nurses are required to spend years on their education to help care for their patients.
