Linda’s case study
Question 1
Long-term care facilities look after patients who suffer from terminal illnesses. In any palliative facility, the wishes and wants of each patient are paramount, especially when they still have the mental ability to make decisions. Linda talked to her caregivers and notified them of her wish to die peacefully, due to the pain she experienced from the three pressure ulcers and her inability to swallow. She also expressed her wish not to be subjected to the gastric tube for feeding. So, the primary issue causing moral distress is the fact that Linda’s mother went against the wishes and took her to a physician who administered the feeding tube. In that, they should have compassion and desire to do good. Linda’s medical state forces her to endure too much pain. Additionally, the inability of her ulcers to respond to medicine is evident that she cannot recover. Therefore, allowing her to die peacefully as per her wishes is not only compassionate but also fair. Therefore, letting her endure more is against the ANA ethics.
Question 3
The main ethical principle that plays a role in the patient’s suffering and staff’s moral distress is autonomy. By putting Linda on the gastric feeding tube, her mother went against Linda’s right to self-determine (Quill & Miller, 2014). So it is the infringement on Linda’s sovereignty that causes distress to the nurses.
Question 4
The main principles that clash between the staff and Linda’s mother are beneficence and non-maleficence, respectively. Allowing Linda to die would be causing harm, which is against the palliative ethical principles. On the other hand letting Linda suffer in pain would be a sign of lack of compassionate.
