Quality Improvement and Accreditation Cognizant of the fact that quality care is less expensive, for a number of years, healthcare organizations have looked for ways to improve quality and operational efficiency while cutting costs. Although American medicine is the best in the world, there are no doubt that there are also waste and inefficiencies in the system that have put health and resources at risk. Patients who stay longer than need in the hospital are exposed to infections. Vital informations are at times lost in transition from primary care doctors to specialists. These are some examples of inefficiencies that require quality improvement.

Integral to a successful quality improvement is the integration of all the organization’s processes and procedures with its quality improvement efforts, identification and prioritization of potential areas for improvement using its patient population, practice operations or even established quality improvement measures as guide, data gathering and analysis to help understand how the its system works, set measurable goals, and monitor the effectiveness of change as well as communicating results. According to the Centers for Disease Control and Prevention (CDC) (2012), the average time patients spend in the U.S. emergency department (ER) before they can see a doctor has increased to 25% between 2003-2009. The main cause of longer wait times (WT) in the ER is overcrowding. Overcrowding has been found to be closely related to both subjective and objective patient satisfaction (Miro’ et al, 2003). Longer wait times in the ED is such an important issue because its consequences are detrimental not only to the ER patients, but also to providers. As a health care provider, decreasing patients’ WT in the ED is essential, although challenging, to improve patient’s health outcomes and increase patients’ satisfaction. Although it is a very challenging issue to tackle, hospitals that have initiated some quality improvement (QI) strategies are experiencing some positive outcomes in that area of care. The outcomes are measured by decreased waiting times, improved patients’ clinical outcomes and increased patients’ satisfaction.

External and internal influences are relevant in health care. These influences continue to affect the total operations of a health care facility. I will summarize the insights I have gained into the external influences of the new health care reform policy and quality initiatives. The recent health care reform legislation was passed in the house and senate this year. The senior vice president, that I have interviewed, states that health care reform is an “unknown” for organizations. In addition, I will research the quality improvement initiatives and how these external influences include implications for organizations and health care administrators.

Ensuring that these processes are implemented in a timely, effective manner can also improve the quality of care given to patients. Management of the processes ensures accountability of the effectiveness of care, which, as mentioned earlier, improves outcomes. Lastly, providing reimbursements based on the quality of care and not the quantity also decreases the “wasting” and overuse of supplies. Providers previously felt the need to do more than necessary to meet a certain quota based on a quantity of supplies or other interventions used. Changing this goal can significantly decrease the cost of care due to using on the supplies necessary to provide effective, high-quality care.
