At the onset of the twentieth century, many changes in American life were occurring, including the expansion of cities, an increase in immigration, and a change in the typical family structure. A movement of social reforms known as progressivism arose to counter these changes, which were viewed as a challenge to domestic life. Specifically, venereal disease caught the attention of many progressives. As members of the medical profession learned more about the pathology of disease, venereal disease became regarded as the ultimate threat to the American family. The biggest outrage was the infection of innocents, women and children who encountered the disease as a result of unfaithful husbands and fathers. Efforts by the progressives to stop the spread of venereal disease were first directed towards members of the military, who were far detached from the wholesome values of home. Soon after, public health measures were extended to the entire United States population. In the early twentieth century, public health campaigns stressing the importance of sexual continence arose to stop the spread of venereal disease. Issues of morality and hygiene surrounding the military became known to the progressives after reports of lewd behavior amongst troops stationed along the Mexican border surfaced. In 1916, the American Social Hygiene Association dispatched Raymond B. Fosdick to investigate such claims, which he soon found to be accurate (Brandt 124). According to records, 288 per 1000 of these troops reported venereal disease, which equates to almost thirty percent (Brandt 127). As US entrance into World War I became more and more likely, the situation became a cause for concern. The disease was viewed as a black man’s disease due to its vast spread in the black race community. In this chapter, it is clear that the medical fraternity had formed opinion of the disease even before the start of the experiment. The theme of racial prejudice is brought out clearly in this chapter. The blacks are discriminated from the whites even after learning that syphilis can affect both races alike. Finally, during World War I, the progressive reformers were able to bypass the Congress in 1918 to create a bill called the Division of Venereal Diseases within the Public Health Service (PHS) (Jones, Bad blood: The Tuskegee syphilis experiment, 1993). In 1932 the United States Public Health Services was responsible for monitoring, identifying, ways to treat sexually transmitted diseases in all US citizens. Public health service was sponsored by Rosenwald Fund; identified Macon County, Alabama had the highest rate of male population infected with the Syphilis. So the Tuskegee Institute was approached to study the effects of untreated syphilis on a black male population for duration of six to nine months and then follow-up with a treatment plan. The research was led by Dr. Taliafero Clark, six hundred Macon County men, 399 with syphilis and 201 who weren’t infected, were enrolled to be part of the study.

One measure they had was that of the “red cross”. This is where they would paint a red cross on the doors of people who were diagnosed with the plague. I think this would have worked because it would let people know who had the plague, so that they could avoid falling ill themselves.
Another guest speaker and guide in this podcast is David Quammen. He talks about how the epidemiologists were trying to figure out what this new disease was and how they were thinking that maybe it was a sexually transmitted disease. So, the CDC launched the study of a group of about thirty patients came from New York, Los Angeles and San Francisco to see who had had sexual contact with who. They led as a series of interviews: “Please name all the people that you have had sex with”. After these surveys the CDC eventually released the results in the form of a diagram, like a network drive with circles representing patients and lines representing sexual contact. In each patient, each circle was numbered so that they could tell who is who. They noticed: New York seven, Los Angeles twelve etc… and soon they noticed a common denominator in this huge spider web of connections. One little circle, numbered zero: PATIENT ZERO. That was the first time they ever used the term patient zero.
In the late 1800’s a group of Americans decided that something needed to be done about the decline of moral and ethical values in most Americans. These people called themselves the progressives and started one of the most comprehensive reform movements in the United States to this day. Progressivism became so widespread that by the end of World War I, anyone who didn’t agree with Progressive ideals was labeled a communist. The Progressives had four major goals that they wished to accomplish. These four goals were to democratize America, to Americanize America, the humanization of capitalism and rationalization of the economy. Each goal dealt with a different aspect of America’s society that the Progressives thought needed help. The way these goals were accomplished was to get laws passed that would reform the practices of many Americans. Progressives held that in order to bring American back to its old time, rural values people would need to attend church more.
After the war, the American people made the change from "old" ways to "new" ways. Many factors, such as new technology, fundamentalism, new looks and church led to tension between the old and the new. The 1920s were a time of conflicting viewpoints between traditional behaviors and new and changing attitudes.
Diet, Health, and Sanitation in Victorian England are so interrelated that it is difficult to examine one without being led to another. A.S. Wohl sums it up when he states: "It is rather commonplace of modern medical opinion that nutrition plays a crucial role in the body's ability to resist disease and the experience of the World Health Organization indicates that where sanitary conditions are rudimentary and disease is endemic (that is, where nineteenth-century conditions prevail, so to speak) diet may be the crucial factor in infection" (Wohl 56). However, there was often a vicious cycle at work in these trying times and it is difficult to point to the root causes of some of the contagion that infected people. Also there were various philosophies, some not as instructive as others, being practiced in the early part of the nineteenth century that tried to explain sanitation problems and poverty. When can see how pervasive this problem was as it made its way into much of the literature at the time. Its representation was rather grim. Works such as Charles Dickens's Oliver Twist and Elizabeth Gaskell's Mary Barton represent the harsh reality of these conditions.

American society was transformed in the 20th century by the economical and social changes stemming from the industrial and urban boom. Many of these transformations included innovations in science, technology, mass communication and transit. This heavily impacted the standard of health and living conditions of many workers in the cities. This transformation would bring about a movement known as “progressivism”. Progressivism is basically a label for a group of individuals, mostly urban college-educated middle class supporters, who demanded reform from the government and businesses to improve working conditions, eliminate health hazards and give the public more control in the government. This movement was brought about in response to the monumental changes due to modernization, the large growth of corporations and railroads and the fear of corruption in American politics.
The aspect of sex and the use of birth control were touchy issues in the early 20th century. Sex was only for married couples that wanted to have children. The idea of sex before marriage was crazy. Because of all the beliefs about sex being only for procreation and not for pleasure birth control was not needed. There was one major event and one key person who are responsible for making the use of birth control acceptable in America. The major event being World War I. In World War I there was a lot of usage of whore houses by the French and English soldiers. A major problem with the French and English governments allowing men to go to the whorehouses was that there was rapid spread of VD. A soldier that is suffering from herpes, syphilis, or gonorrhea doesn’t fight too well. President Wilson was very concerned about the American soldiers getting sick and not able to fight. He also wanted to stop the spread of VD Wilson decided that during boot camp the soldiers would be taught about VD’s, and the effect of them. The recruits had to take a sexual education class. In the class the men were shown pictures and movies that showed what various vaginal diseases looked like, and what they would do to a mans sex organs. This was an effective in stopping the American soldiers from going to the French whorehouses. Another program the U.S. Government instituted to stop the spread of VD was to give out condoms to the soldiers.
