Program Evaluation Paper

During and post the data collection phase of this program evaluation; the program evaluation staff will compile the data in several ways. The program evaluation will be relatively brief given the small number of program participants, staff and financial amounts of financial information involved. No interim reports will be necessary as the process will be succinct. There will only be one email sent to the Executive Director and Chief Financial Officer, advising them the questionnaires have been sent out and the evaluation staff is waiting for their responses to proceed with that portion of the evaluation.
Before mailing, the evaluation staff will ensure the charts contain a signed consent form allowing them to be contacted for program follow-up charts of former patients/clients in the Ohel-Bais Ezra Outpatient Substance Abuse Program. Once verified that the charts contain a signed consent form, questionnaires will be sent out via postal mail to all past program

A meeting with the Executive Director and Chief Financial Officer will be scheduled to discuss the findings and at that time, a subsequent meeting with the rest of the administrative staff of directors scheduled. The complete written summary will be presented to all in attendance. A presentation will be given, questions posed by meeting attendees answered by the program evaluation staff and recommendations for outpatient substance abuse programming. Should it be requested, the program evaluation team may meet with the Ohel-Bais Ezra Board of Directors to present their findings (Greene, 1988). The Ohel-Bais Ezra administration may not request or desire this meeting, although it might be beneficial for the organization and funders as a whole. This would require an additional expense as this was not calculated into the previously submitted program evaluation
