Among the issues or challenges that were identified on Staffordshire NHS Foundation Trust Public Inquiry I would like to focus on “high degree of tolerance of poor standards of care and tolerance of risk to patients”. Patients and visitors have the right to receive quality health care free from risks and violence. Health personnel and leader must be able to identify, assess and eliminate risks, errors, negligence towards patients care. They have to provide an appropriate response and action where error or violence occurs. Poor standard care should not be neglected nor tolerated instead need to be proactively identified, assessed, report and addressed which leads quality care and patients satisfaction (Victorian Government Initiative 2015). Risk management activities must review time to time by leaders and externally methods to improve patient safety and quality care. There must be access to information necessary for risk management process.
