Mrs. P., a 48-year-old, African-American female with a diagnosis of uncontrolled hypertensive urgency. She had a history of various medical conditions, including diabetes mellitus type two with neuropathy, hypertension, hyperlipidemia, acute kidney injury, intermittent headache, and iron deficiency anemia. The patient’s blood glucose level and blood pressure were remarkable elevated (FSBS 280 mg/dl and BP 190/130 mm Hg). Unfortunately, due to the lack of health insurance, Mrs. P. did not follow prescribed health care plan. The patient could not recall the last time she took her medications to regulate elevated blood glucose level and high blood pressure. In order to deliver a high-quality patient care, a nurse has to establish effective working relationships with each of the interdisciplinary team member. The nurse is often a clinical leader who coordinates patient care among the team members. Effective communication between health care professionals is essential in providing comprehensive, organized, and patient-oriented treatment. Sometimes, poor communication (omitted relevant information) between healthcare personnel can jeopardize patient’s safety. P.’s diabetes ulcer. The patient’s CRP (3.8 mg/L) and ESR (35 mm/hr) were remarkably elevated which is indicative of existence of infection. X-ray revealed mild periosteal change of her left great toe. She has been prescribed a course of antibiotics (Vancomycin and Zosyn) to eliminate the infection and preserve the left great toe. The podiatrist was thankful to my preceptor, Dora for catching the problem on time, otherwise Mrs. P. could lose her left great toe. Lastly, we made a request for the diabetes education to give the patient a better understanding of a disease management and noncompliance consequences. Additionally, a social worker provided resources to obtain affordable health insurance and referral to the SGMC Diabetes Management Clinic to receive an appropriate care. Self-Evaluation of Learning From this particular situation, I learned that nurse is indeed a key factor in the interpofessional communication and collaboration. The nurse comprehensively assesses, identifies an immediate or potential issue in a timely manner and communicates to the appropriate health care personnel.
