Pneumonoultramicroscopicsilicovolcanoconiosis, also known as Black Lung Disease, is a unique form of pneumoconiosis and silicosis. Pneumonoultramicroscopicsilicovolcanoconiosis is a respiratory disease that affects the lungs caused by the inhalation of silica dust causing inflammation in the lungs.The crystalline silica dust that causes this disease is most commonly referred to as quartz, but it is also found in sandstone, granite, slate, coal, and pure silica sand. The risk factors that Jessica presented with are a history that is positive for smoking, bronchitis and living in a large urban area with decreased air quality. The symptoms that suggest a pulmonary disorder include a productive cough with discolored sputum, elevated respiratory rate, use of the accessory respiratory muscles during quite breathing, exertional dyspnea, tachycardia and pedal edema. The discolored sputum is indicative of a respiratory infection. The changes in respiratory rate, use of respiratory muscles and exertional dyspnea indicate a pulmonary disorder since there is an increased amount of work required for normal breathing. Tachycardia may arise due to the lack of oxygenated blood available to the tissue stimulating an increase in heart rate. The pedal edema most probably results from decreased systemic blood flow.
The presented case is of a patient named R.S. who has a smoking history of many years, which can be directly tied to his development of chronic bronchitis, a chronic obstructive pulmonary disease (COPD) specified as Type B. It is estimated that in 90% of chronic bronchitis or “blue bloaters”, cigarette smoking is the major cause. Chronic bronchitis involves persistent and irreversible airway obstruction, due to the constant inflammation of the bronchial mucosa, leading to hypertrophy and hyperplasia of bronchial glands. The latter exposes the individual to higher risks of bacterial infections; often colonization of organisms such as Streptococcus or Staphyloccocus pneumoniae can be exhibited. This is due to the lost or impaired function of mucociliary clearance action which results from the replacement of certain sections of ciliated columnar epithelium by squamous cells in the bronchi. This exposer caused side effects such as chest tightness, shortness of breath, long term respiratory diseases (COPD), and permeant loss of lung functions. Slurry, the residue which is used to clean the coal can wash into groundwater and may contain arsenic, lead, manganese, iron, sodium, strontium, and sulfate. In reality, it is the opposite with life and death on the line due to unnecessary slaughter between human beings because they cannot solve their own conflicts. This devastating topic is told by Joseph Boyden’s novel "Three Day Road" by two different protagonists sharing their own adventures and their conflicts. The protagonist Xavier and antagonist Elijah experience bloody warfare ability to cause havoc. They begin to understand that bloody warfare can make them physically and mentally draining over time because how it changes a person personality making them corrupt with intent to become a windigo themselves like Elijah did, striving to kill another human being as their only goal for survival. As humans, Joseph Boyden is reflecting that if the readers were Xavier or Elijah themselves place in that unstable environment filled with diseases and distraught. We would experience the same kind of suffering Elijah and Xavier had to go through.

Chronic bronchitis is a disorder that causes inflammation to the airway, mainly the bronchial tubules. It produces a chronic cough that lasts three consecutive months for more than two successive years (Vijayan,2013). Chronic Bronchitis is a member of the COPD family and is prominently seen in cigarette smokers. Other factors such as air pollutants, Asbestos, and working in coal mines contributes to inflammation. Once the irritant comes in contact with the mucosa of the bronchi it alters the composition causing hyperplasia of the glands and producing excessive sputum (Viayan,2013). Goblet cells also enlarge to contribute to the excessive secretion of sputum. This effects the cilia that carry out the mechanism of trapping foreign bodies to allow it to be expelled in the sputum, which are now damaged by the irritant making it impossible for the person to clear their airway. Since the mechanism of airway clearance is ineffective, the secretion builds up a thickened wall of the bronchioles causing constriction and increasing the work of breathing. The excessive build up of mucous could set up pneumonia. The alveoli are also damaged enabling the macrophages to eliminate bacteria putting the patient at risk for acquiring an infection.

Pulmonary Fibrosis is a condition where the lung tissue becomes thick and scarred. The thickening and scarring of the lungs makes it hard for the oxygen supply to be delivered throughout the body. The scarring can be caused by many different factors, but it is hard for doctors to figure out exactly what caused the onset of this disease. The damage caused by this disease cannot be repaired. Pulmonary Fibrosis usually affects the age group of forty to seventy years old. Men are more likely to develop this disease, but women can also get this disease. Pulmonary Fibrosis is not a transmittable disease. Little is actually known about how the disease develops. There seems to be a genetic connection and environmental factors that cause the disease to develop.
