I have communicated with my family several times since my first visit. I have seen them in person for my ECSE visits, talked to B on the phone a couple of times, and texted B on several occasions. The family wanted me to do my second visit during their morning “hanging out” time. I went on a Monday, so M was in school. B and X were in the living room playing with X’s turtles and watching TV. J joined us shortly after I arrived. B told me that she and X have started doing a craft every day. X used to have a hard time sitting during meals, so a couple of months ago, the family purchased a child-sized table and chairs for him and one for M. X’s table has Ninja Turtles on it and he really likes it. He now sits at it during meals and uses …show more content…
This team is comprised of a Services Coordinator (SC) physical therapist (PT), occupational therapist (OT), speech/language pathologist (SLP), and an ECSE teacher. The family began working with the EI team about 18 months ago. B first found out about EI when I met her while she was working at the school’s summer program. She told me about her concerns with X. B said that they were currently traveling to another town to for therapy for him and that it was extremely difficult for the entire family. Once X qualified for EI, B and J were able to stop making the trips out of town for therapy. B told me that the collaboration with the EI team has been positive because the providers come to their home which allows X to remain in the environment where he is most comfortable. B also said that the EI team really listened to J and her and valued their input regarding X. She talked about how two team members came to their house early in the process and asked her all about the family’s day and discussed which parts of the day were most difficult. She said that she and J were able to decide which goals would be most helpful for X and their
The IEP team may include the student, their parents, a regular teacher, a special education provider and other representatives, such as a social worker or relative child care provider. These meets are required to be held within 30 days of the student’s acceptance into the special education program. Every IEP has the two main goals of setting reasonable learning goals and establishing academic services that the school will provide. The IEP should state which state and district-wide assessments that the student will or will not participate in and why.

This is a vital skill to remember as patients family/loved ones can often feel unimportant and distant toward nursing staff, and no one knows the patient better than they do, and can tell you vital information. Possible Outcomes: A meeting will be planned with the Special Education teacher, General Education teacher, Principal (or district representative) and individual who can evaluate the child. We will work on goals for Tiffany and set those goals with a yearly IEP and a 3 year revaluation. Tiffany will receive a new annual goal and her schedule will most change. Tiffany’s schedule may be altered and she will be in the general education room for whole group instruction and for a portion of the day the special education room for academic support. I observed an IEP meeting for one of my students who has a TBI, traumatic brain injury. She just moved to this school district last year, so this was a follow-up IEP meeting to see how she was doing on her short-term objectives and overall goal. They also talked about transitioning her into high school, because she is in 8th grade now. The People who attended the meeting were the DAPE teacher, the mom, the student, the three special education teachers, the para-professional, and case manger.
My family had moved to Oregon when I was 7 into hunter’s run apartments and we moved AGAIN when I was 9 to SW Bonnie Meadow Ln, into a fairly nice house. Care coordination within health care systems ensures the client of an effective and short stay. Care coordination refers to the coordination between and among professional teams that serve valuable roles involved in providing care to clients. Different disciplines of health care professionals include nursing, medicine, case management, pharmacy, nutrition, social work, and allied health professionals, such as speech therapists and physical therapists. They are found in all health care delivery systems and are extremely effective when the focus is strictly on the needs of the client. Interprofessional teams are valuable because each health care professional has specialized knowledge and skills so that health care plans are determined with patients’ best interests in mind. With the communication of ideas amongst the disciplines, their roles consequently complement one another in an age of exponentially growing information. This team process of care coordination can improve quality of care, enhance client satisfaction, and reduce hospital cost by decreasing length of stay; care coordination ensures the best possible outcomes (Koch, 2014, p. 436).
The collaborative team did a great job interacting during both cases. They were very professional, and respectful to one another. Each member of the team played a significant part in each procedure.
Decisions for advanced care plans have been communicated and discussed with patient and caregiver to alleviate stress of decision-making and remove pressure from the family members.
Most parents know, or at least have a general understanding of the environment or atmosphere that their student needs be in to succeed. The district encourages that parental input be given to the IEP team during a multi-disciplinary meetings, and always tries to take that input into consideration. However, sometimes what the parent wants is not always in line with the student’s ability level, and the IEP team will try to find a compromise, or even offer trial periods to evaluate progress and behavior (S. Cummings, personal communication, November 12th, 2015).

Inclusion: Patients’ family members, relatives, friends, and partners should also be included in the process of care and support as it fosters mutual relationships among them and also care outcomes.
I would also have to explain to the informant that their information will have to be passed on in confidence, in order for Linda to receive some help. The information would be shared with the multi-disciplinary team, which in respite care could include the family doctor, public health nurse, social worker and clinical psychologist. They would discuss the best possible ways in which to approach and treat Linda’s problems involving her family and also liaising with her school.
