I will be discussing the three CLAS standards have selected and determine how these standards are applied at my establishment. I will write recommendations of ways that my establishment can improve on these three CLAS standards I selected with supporting examples. The first CLAS standard I picked was to inform all individuals of the availability to language assistance services in their preferred language, verbally and in writing. I do not work for a hospital, but a long term care facility with short term rehabilitation so the manner in which this is conduct will be vastly different than a hospital. New hires and quarterly the staff are given in-services in regards to discrimination, abuse and neglect. We do not have a specific in-service in regard to language and culture and what to do. I did ask some staff members what would happen if someone spoke no English and they came in as a resident how they would communicate with them and know about their culture. The response was from I do not know, to the family to hand gestures. The staff stated they have not encountered at this facility, but in places they worked in the past when there is a language barrier it takes much more time to communicate the basics to the residents. I think a great opportunity for education and training is to incorporate strategies with the orientation process, and quarterly in-services as well as needed. Another idea is at monthly meeting review different cultures that we can pull from the demographics of the area and discuss cultural differences and care needs. By posting information up prior to the meeting the staff can perhaps look up information about the culture and answer questions for prizes. Also serving a traditional dessert or small food items will also make it entertaining for the staff. By making cultural differences fun and interactive the staff will better embrace when someone of a different culture admits to the facility. The section where this CLAS standard is located places that the responsibility for implementation at the highest levels of organizational leadership (Koh, Gracia, & Alvarez, 2014). The Quality and Safety Education for Nurses (QSEN’s) goal is to prepare future nurses with the knowledge, skills, and attitudes (KSAs) that are needed to continuously improve the quality and safety of the healthcare systems within which they work. QSEN focuses on six main competencies; patient-centered care, teamwork and collaboration, evidence-based practice, quality improvement, safety, and informatics. As we have learned in earlier classes these competencies and their KSAs offer a base to help us and other nurses as we continue our education and become RNs. As we will learn in this class these KSAs go hand in hand with health assessment.
While working seemingly endless days, many nurses do not realize the many influences that affect their professional practice or how client care is delivered. Besides their employer, health care organizations are highly regulated by federal, state, and local laws and regulations. In addition to the rules set by governments, most medical establishments want to be accredited by The Joint Commission (TJC), a non-government regulatory agency. TJC does not have the authority to cite or fine a facility for not meeting standards or responding to its custodian alerts (The Joint Commission, 2011). However, these standards carry considerable weight through the loss of millions of dollars from Medicare and Medicaid programs.
Q.3 Nurses as part of regulated health care practitioners are responsible and accountable to abide by the standards, codes and guidelines of nursing practice (NMBA, 2016). The nurse in the case study has breached the standard 1.4 of the Registered Nurse Standards for Practice. According to standard 1.4, the registered nurse should comply with "legislation, regulation, policies, guidelines and other standards or requirements relevant to the context of practice” when making decisions because this will be the foundation of the nurse in delivering high quality services (NMBA, 2016). The nurse in the scenario did not follow the hospital policy concerning “Between the Flags” or “red zone” and a doctor should be notified of this condition. Furthermore, the nurse failed to effectively respond to a deteriorating patient.
Standards are important aspects of nursing that a nurse must learn and implement every day for the rest of their nursing career. These standards provide for a nurse’s competence in the quality of care they deliver to the public. Standards offer a necessary guidance to nurses everywhere in an effort to ensure that people are treated correctly and ethically. Patients expect nurses to have a general knowledge of the medical realm and to know exactly what it is they –as nurses- are responsible for. Nurses need to have a sense of professionalism that enable the patient to feel safe and secure, knowing that a competent person is caring for him. A lack of professionalism does the opposite, making it impossible for a patient to trust or respect the nurse caring for him. Standards of nursing, if utilized correctly, give the nurse that sense of professionalism the patient is expecting. It insures for the safety of the patient and allows the nurse to provide quality health care that is expected of a medical professional.
Providing culturally competent care is a vital responsibility of a nurse’s role in healthcare. “Culturally competent care means conveying acceptance of the patient’s health beliefs while sharing information, encouraging self-efficiency, and strengthening the patients coping resources” (Giddens, 2013). Competence is achieved through and ongoing process of understanding another culture and learning to accept and respect the differences.
“Cultural awareness is the foundation of communication and it involves the ability of standing back from ourselves and becoming aware of our cultural values, beliefs and perceptions (Quappe, 2007). It is important to be aware of what culture your patient is from because than the nurse can give the patient and family the care that is not offensive o their beliefs. Cultural sensitivity Cultural sensitivity is experienced when neutral language–both verbal and nonverbal–is used in a way that reflects sensitivity and appreciation for the diversity of another. It is conveyed when words, phrases, categorizations, etc. are intentionally avoided, especially when referring to any individual who may interpret them as impolite or offensive (Giger, 2006). Being sensitivity to ones cultural is imperative, this allows the patient and or family to feel more comfortable and have their personal beliefs be respected. Different forms of cultural sensitivity include choice of words and language used while talking to a patient, the use of space while communicating, as well as who in the family to talk to and how to address patients and family. Cultural competence is a combination of the skills, knowledge and attitudes that are needed to deliver the proper excellent care to a diverse population. Culturally competent care is care that respects diversity in the patient population, and cultural factors that affect health and health care, such as language, communication styles, attitudes, behaviors, and beliefs. The national CLAS Standards provide the blueprint to implement such appropriate services to improve health care in the United States. The standards cover many areas, such as leadership, workforce, governance; communication and language assistance; organizational engagement, continuous improvement, and accountability. (Agency for Healthcare Research and Quality, 2014).
By understanding the way a culture factor influences nursing care, we are able to improve our teachings, improve understanding and provide overall better care for a healthier outcome for our patients. An example of this could be having a staff member that is bilingual or teaching materials written in a different language. According to Andrews & Boyle (2016), the first step is for the nurse to do a self-assessment of themselves in order to identify their beliefs toward people with different backgrounds, which will provide clarification of possible unknown behaviors. The second step is to set goals for the patient and those involved with the patient’s care.
The Professional Standards of The College of Registered Nurses of British Columbia,(CRNBC) include the following four key areas: Professional responsibility and accountability, Knowledge-Based Practice,Client-Focused Provision of Service and Ethical Practice(p,2). Each of these standards are outlined well within the CRNBC professional standards, and need to be followed by each registered nurse. If these standards of care are not being met then one must evaluate their own practice or it could need to be reported to CRNBC. This paper will explore three standards of practice and their importance, including: Challenges, Professionalism and Knowledge Based-Practice. These reflect significantly on registered nurses (RN) in Canada, and how they play their role as nurses professionally.
The nursing code of ethics has a very standard definition. It is the base on how nurses should guide themselves in conduct by making the right decision regarding ethical issues. According to the National Student Nurses Association “students of nursing have a responsibility to society in learning the academic theory and clinical skills needed to provide nursing care” (2003). In the clinical setting nurses have a lot of responsibilities while caring for an ill patient, they have the obligation to practice their profession with compassion, love, and respect the uniqueness of each patient, as nurses we are not supposed to deny care to a patient because of their economic status, their skin color, race, or the nature of health problems, we are here to help the people in need in particular those of susceptible populations. The NSNA states that the code of conduct is based on an understanding that to practice nursing as a student is an agreement that trust and honesty is depended on us by society. The announcement of the code provided direction for the nursing student in the personal development of an ethical foundation and not limited to the academic or clinical environment but can assist in the holistic development of a person. As a nurse strive to provide culturally sensitive care, they must recognize how their client's and their perceptions are similiar as well as different. Nurse enhance their ability to provide client-centered care by reflecting on how their beliefs and values impact the nurse-patient relationship. To provide appropriate patient care, the nurse must understand her/his culture and that of the nurse profession. Cultural biases can be particularly difficult to identify when the nurse and client are of a similar cultural backgroup. When we recognize and know a culture, we will know what is right for our patient, and thus may impose our own values on the client by assuming our values are their values. Recognizing differences a present an opportunity not only to know the other, but also to help gain a greater sense of self. Characteristics can be as diverse as ethnic background, language spoken, gender status, physical appearance, race, and religion to name a few. Migration from various countries is creating a diverse population with different cultures and languages within the United States. Due to these cultural differences and lack of knowledge, disparities are increasing. Studies have shown that both language barriers and lack of cultural customs can hinder the services provided to the patient by the healthcare worker (Renzaho, Romios, Crock, & Sonderlund, 2013). This study provided a positive outcome when communication and cultural mutual understanding took place and patients had a more positive health outcome. It is very important that nurses are diversified in various cultures in order to better care for our patients. According to Mareno and Hart (2014), cultural competency has become one of the core values being taught in nursing programs. Their study showed that the perceived level of cultural awareness and skills among the nurses provided was low. Awareness and knowledge levels increased with higher education. It was highly recommended that self-awareness exercises be incorporated into the nursing course and continued to be addressed during the remaining curriculum until
