Liaison: As part of my day to day duties, I frequently liaise with both internal and external professionals and organisations to improve patient’s quality of care. Eg: GP, social service team, care home staff, care agency team, district nurses, Single point access team, specialised consultants, orthotic department, stroke association, Age UK, LIFE (reablement team), neuro navigator etc.. Liaise with ICB /CCGs: When I was working as part of a nursing home, I had experience of attending ICB /CCG meetings on a regular basis to support the assessors to take decisions based on patients continuous heath care funding. Writing therapy report to ICB/CCG based on patients current state. Working closely with Neuro-navigators to deal with level 1 rehabilitation referrals. Clinical Governess: • Provided Trainings: Experience in providing training for different staff members including therapy team members, caregivers (such as HCAs) and carehome/ nursing staffs in a regular manner Eg: Training about different fall prevention equipment’s/ measures, splint training, manual handling training, detailed case study discussions especially for therapy team members
