Module 1: Patient Advocacy Healthcare is an ever changing field. It grows and flows with the changes in society and the influx of new technology. In the early years of professional nursing basic needs of cleanliness and rest laid the foundation for the current advancements in infection prevention and safety. In this paper, the basis for safe practice will be identified thus allowing the nurse to work towards maintaining and improving the safe treatment of the patient. Of high importance in this process is to lay the foundation for open communication. This can be accomplished in the form of safety huddles, collaborative communication, message boards, and/or internal email notices. The Joint Commission (TJC), whose ultimate goal is to promote quality of care in which safety emerges from, assists by creating standards for health organization to follow. Their “do no harm” motto is at the core of the guideline principles: 1-reduce harm, 2- recommend methods to staff that will improve quality of care and safety and 3- encourage non-punitive discipline methods to increase accountability, knowledge and trust for caregivers (TJC, 2018). In response to TJC surveys, this nurse has witnessed several process implemented to ensure patient safety. A few of these processes are education competencies, safety always incident reporting, daily huddles at the “Visibility Wall”, and biannual skill fairs. Helping to champion nursing issues takes place directly with decision-makers on Capitol Hill, too. ANA believes in empowering nurse to make a difference in their profession by sharing their viewpoints and expertise with law makers. This nurtures the nurses to become more politically engaged. Nurses are central to patient care and patient safety in hospitals. Their ability to speak up and be heard greatly impacts their own work satisfaction and patient outcomes. Open communication should have been encouraged within the healthcare team caring for Tyrell. Safety competency is essential for high-quality care in the medical field. Nurses play an important role in setting the bar for quality healthcare services through patient safety mediation and strategies. The QSEN definition of safety is that it “minimizes risk of harm to patients and providers through both system effectiveness and individual performance.” This papers primary purpose is to review and better understand the importance of safety knowledge, skills, and attitude within nursing education, nursing practice, and nursing research. It will provide essential information that links health care quality to overall patient safety.
Nursing advocacy is a professional obligation and the standard of practice expected by the Ontario College of Nurses (CNO, 2009). The concept of advocacy is enshrined into the code of professional ethics that nurses “must promote the interests of clients in their care” (CNO 2009). To meet this standard the nurse must first ensure a deep understanding of advocacy and how it relates to the nursing profession. The nursing profession is a profession where people put their trust in you to provide care that is not only effective, ethical, and moral, but safe. Not all health situations are simple or by the book. Not all hospitals have the same nurse-patient ratios, equipment, supplies, or support available, but all nurses have “the professional obligation to raise concerns regarding any patient assignment that puts patients or themselves at risk for harm” (ANA, 2009). When arriving at work for a shift, nurses must ensure that the assignment is safe for not only the patients, but also for themselves. There are times when this is not the situation. In these cases, the nurse has the right to invoke Safe Harbor, because according the ANA, nurses also “have the professional right to accept, reject or object in writing to any patient assignment that puts patient or themselves at serious risk for harm” (ANA, 2009).
Although our main mission is to provide great care to our patients, Florence Nightingale has taught us (Burkhardt & Nathaniel 2008, 3rd Ed. P18-19) that if we do not have legislative power many of our ideas can be oppressed therefore prevent the profession from thriving. Merriam-Webster’s online dictionary defines an advocate as “one that pleads the cause of another,” or “one that supports or promotes the interest of another” (2014). Bu and Jezewski expanded on this by developing a mid-range theory of patient advocacy to implement into nursing (2007). They describe a patient as being “vulnerable and powerless” when stricken with illness and without knowledge of health care systems (Bu & Jezewski, 2007). The American Nurses Association (ANA) developed a foundation for which all nurses are expected to perform their basic duties in order to meet the needs of the society we serve. Nurses are at the forefront of the healthcare system and therefore must actively engage in policies that impact the healthcare system. DNP graduates can initiate the process of change by pointing out potential problems within the healthcare delivery system and spearheading legislation through lobbyists and nursing organizations. In Nursing, there will always be instances where the patient's nurse needs to advocate for their patient. There are numerous reasons why a nurse would advocate for their patient ranging from getting the doctor to change the patient’s orders, helping the patient’s treatment team understand what it is the patient is requiring for the day, to expressing the patient’s last wishes before death. In every situation, the nurse should do what is in the patient’s best interest. Tomajan (2012), “Advocacy skills are the ability to successfully support a cause or interest on one’s own behalf or that of another. Advocacy requires a set of skills that include problem solving, communication, influence, and collaboration”(p. 2). With those skills, the nursing staff will be able to work together to advocate for their patients. Along with those skills, nurses need to keep in mind the three core attributes that are: safeguarding patients’ autonomy; acting on behalf of patients; and championing social justice in the provision of health care. Patients Safety is the most crucial about healthcare sector around the world. It is defined as ‘the prevention of patients harm’ (Kohn et al. 2000). The definition of advocacy is the act of speaking on the behalf of or in support of another person, place, or thing. In its simplest form it means standing up for what one believes in for both self and others. Nurses may act as an advocate by supporting knowledgeable decisions, by acting as liaisons or by interceding for another individual. The objectives of a nurse as an advocate are to notify, enhance independence, and respect the decision of the patient (“Patient, Subordinate and Professional Advocacy, n.d.). Simply put, the nurse is most often the go-between or mediator between the healthcare system and the patient. Advocacy is mannered by the nurse’s code of ethics. These codes impact the profession across-the-board. While certain codes may vary from country to country, the central principles are created for the relationship between the nurse and the patient.
