7. Self-Assessment & Assessment Form: (Due 1/9/18) Thoroughly fill out these two forms based on knowledge of yourself and return in the envelope

DHCD Monthly Re-housing Plan: (Due 1/9/18) This is a plan all participants (like the budget worksheet) complete monthly, with short to long-term goals, which they sign agreeing to do within 30 days, therefore, the goals are carefully thought out. Your goals will be to complete the items on this list and return all that is required. Currently your December plan is about past due, so please assist by returning these items as quickly as possible and someone will put together with you, January’s plans. This is a lot of information, but please keep in mind participants work and cooperate with a Case Manager who assist them step-by-step through this monthly process. You’ll notice some items are due the 9th of January, all others can be submitted by the middle of January to give you more time. Let us know if you have any questions and someone will help you. Thank you for your help with getting this done. Your next monthly rehousing plan/budget sheet will be due the end of January. In this assignment I will be analysing the purpose of assessment whilst demonstrating my understanding of the different assessment methods used. I will also be giving a brief explanation of my understanding of VACSR describing my understanding of what each element means and its importance when used in assessment.
