6.1 IMPROVEMENT IN METHADONE MAINTANENCE TREATMENT
6.1.1 INTRODUCTION:
Generally, methadone maintenance treatment (MMT) requires the collaboration and support from all areas including community, providers and government. In health care setting, it involves the integration, coordination and sharing information between different health care professional which including doctors, pharmacist and nurses. In community aspect, the perception regarding the heroin addicted patient’s must be eliminated as this will give major obstacle towards the successful of the therapy. All of this will eventually increase the possibility of achieving the intended outcome in both providers and patient perceptive [Gulliford M, Naithani S, Morgan M, 2006].

According to British Columbia Methadone Maintenance Treatment Program, it is suggest that in order to receive authorization to prescribe methadone for maintenance, a physician must complete a one-day workshop and two half-days of preceptorship. The methadone program at Rosthern Hospital is a very active and intense program. Boast over 20 patients that are regular methadone users, the involvement of the physicians, pharmacists, nurses, and addiction counselors are key (Melle, 2016). Dr. Melle is the coordinator of the methadone program at Rosthern Hospital. His roles include, patient recruitment, patient care management, follow up care, and most importantly prescription and dosage of the methadone (Melle, 2016). Pharmacist’s role in methadone treatment is providing the methadone to the hospital, as well as in Warman pharmacy they monitor and dispense some methadone to some of the patients in the methadone program. Addiction counselors are an outside resource that the staff at Rosthern hospital can utilize. There are not social workers or counselors at Rosthern, but Dr. Melle and the nurses prefer if their patients are regularly seeing an addiction counselor while in the methadone program. Finally, the nurses role in the methadone program is early treatment and monitoring of patients during detox, the continued monitoring or patients when methadone doses change, dispensing methadone and monitoring our patients in the recovery program, and observation of patient’s success during the program. With the growing number of individuals addicted to these narcotics, the need for better therapies and treatments to end these addictions has grown exponentially. Of these therapies and treatments prescribed to these patients, the newest and seemingly most celebrated is a treatment known as Suboxone therapy. This therapy is for those individuals who have opiate addictions and are seeking help toward a more speedy recovery (Thompson-Gargano, 2004).

Doctors can also tell if one of their patients has had treatment or is in the process of having treatment. Doctors or families are the ones to refer their patient or family member to a treatment center (“Treatment and Drugs”). There are different types of therapy for a patient suffering from prescription drug abuse. Treatment is usually a 12-step program, but there are other programs included. The type of therapy used is “Group Therapy” where they talk about their recovery with a group of people.
Drug use and abuse has become a common situation with today’s citizens. Drugs are more easily accessible today than they ever were in the past. The drug trend is frightening to say the least. From the legal substances, such as alcohol and tobacco, to the illegal substances, such as marijuana and cocaine, there has been an increase in their use amongst all ages of people. It seems that the ages of the users are getting younger as time goes by. Because of this trend there are more people addicted to substances and who need treatment. There are many treatment programs out there where people can go to get help with their addictions. Programs range from alcohol treatment to substance abuse treatments and detoxification processes. New treatments must be made by realizing drug dependence coexists with habits that must be changed. This must, and can happen with long term supportive health with both clinical practices, and new-evidence based treatments such as Buprenorphine/naloxone rather than Methadone. This type of intervention will work in an extraordinary way with addicts and their sobriety. One major issue that affects the US today is the use of illegal drugs. One of the fastest growing illegal drugs is methamphetamine. Methamphetamine, also known commonly as meth, is "the label given to a homemade substance that is manufactured (typically) in rural labs using fertilizers, cold tablets, and household acids" (Armstrong, 2007, p. 427). When it is manufactured, it is commonly seen as a powder or crystal that is white in color, has no distinguishable odor, and is pungent to the taste (McKinley & Fink III, 2006). There are several means by which meth can be consumed. This can include the product being smoked, inhaled, swallowed, or injected by needles (McKinley & Fink III, 2006 & O’Connor, Chriqui, & McBride, 2006).

One of the largest concerns within our nation is opioid dependence and the rise in abuse rates of medicinal and illicit opioids. Many illicit and medicinal drugs such as Morphine and Heroin, come from the internal secretions of the opium poppy plant (Papaver somniferum) and are highly available and acclaimed for their analgesic and euphoric effects within the general population (Veilleux et al., 2010). Veilleux et al. (2010) concluded that given these properties of opioids there is a dangerously high potential for abuse. Dependence on these drugs can be linked to a variety of aspects within an individual’s life such as health and social problems, which may include increased chances for legal issues, mortality, unemployment, psychotic episodes, and HIV. For these reasons and many others, Opioid addiction is considered a chronic disease with high potential for relapse and needs to be considered as a public health concern.
Drug and alcohol abuse has become a worldwide epidemic within today’s society. The battle against drugs and alcohol is not going to diminish. Therefore, we as a society need to work together to address these problems while incorporating successful treatment plans and services for these individuals. The addiction to these substances does not only effect oneself, but can also have profound consequences for the children and families.
For inmate who struggle with drug abuse, the outside world may provide challenges to getting the right medical attention. Programs should be such that they provide links for the affected to continue receiving medical attention even after serving their terms. This help in reducing their re-addiction. Additionally, a program can recommend the best place to get advisory services or the best place to get services. This process has entails of case management and is perfect example of a good drug related problem follow up.
Substance abuse is an issue that plays a major social problem in society, the effect of drugs on families and communities is destructive.
