1. Does it ask all of the necessary questions to determine what the client’s presenting problem is? Please support your answer. After reading through the entire intake form for the Northside Mental Health Center, I would say that it does ask most of the necessary questions to determine the reason for the client’s visit. There are questions related to substance abuse, eating disorders, and even gender identity and sexual functioning. In saying that I think the questionnaire asks most of the necessary questions, I implied that it didn’t ask all of them. One example of a missed question relates to prior diagnoses. There is a question asking the client to state where they have been treated in the past, why they were there, and to describe whether it was inpatient or outpatient, but after double-checking the questionnaire, there is not a question asking what diagnoses the individual already has (Northside Mental Health Center intake questionnaire, p. 2). Another example of a missing question relates to current prescriptions. One question on the form asks about over the counter medications that are being taken, but there is not a single question asking about prescription drugs that are being taken (Northside Mental Health Center intake questionnaire, …show more content…
Considering it is an intake questionnaire for a mental health center, there are very few questions related to the mental health of the client. As mentioned above, they ask questions related to eating disorders, substance abuse, and a potential gambling problem, but there aren’t any questions about mental health diagnoses or the associated medications. In fact, the only question expressly stating it’s relativity to mental health is the question asking about previous treatment placements. I’ve never had mental health issues, so have never had to fill out an intake form, but those seem like fairly important questions to have been left off of the

...h the inventory is very easy to use and is self explanatory, it’s seems important to evaluate when and why the test is being used with the client and how the results are going to benefit the client. Because the assessment is a self-report assessment, it’s so crucial to help the client understand how important an honest evaluation of their symptoms is to an accurate score.
On May 25th 2016 I officially started an internship with Montgomery County’s Department of Mental Hygiene. The department consists of one hard working woman named Sara Borenko who was my supervisor and boss throughout this internship. One of the main duties of Sara’s job is the funding of community programs that are aimed towards helping the mental health community. In fall 2015 I took a class called Community Psychology and while working at this internship; I applied what I had learned from that class and used it to my advantage. Some of the lessons I applied included social oppression, community organizing, stress and coping, and emotional support. Before taking this internship, I didn’t realize how much my county had to offer as for services. I’m inspired by the community and its strides towards helping the mentally ill. I’ve learned a lot during this internship. I’ve gained experience in the field, I’ve grown a broader
Discharge planning for these patients into the community does not consider living in shelters and assisted living facilities. The revolving door situation creates negative outcomes for patients and organizations. These organizational alternative housing services do not provide medication supervision, therefore adherence to medication regimen and treatment is not provided to them. Consequently, they return to hospitals seeking follow-up treatment. Unfortunately exacerbation of mental illness symptoms is inevitable reflected from premature discharge and inadequate follow-up care. It is imperative that all patients receive proper treatment and discharge plan. Providing discharge instructions to patients for transitioning to independent living and self-care is not sufficient. Proactive health interventions and planning organizational strategies are needed to improve health outcomes for patients who are living in health care institutions (National Health Care for the Homeless council,

The emotional health and wellbeing for our children should be the greatest importance to a parent. Four million children and adolescents in the United States live with serious mental disorders (Hamburg, 2006). Some conditions include depression, anxiety disorders, which are the most frequent that may later on lead to drug abuse. Some develop eating disorders that include bulimia and anorexia, others develop attention deficits, become hyperactive or antisocial behavioral responses that may limit the ability to think and learn and form social attachments or communicate with others. According to the World Health Organization (WHO), mental health disorders are one of the leading causes of disability worldwide. Three of the ten leading causes of disability in people between the ages of 15 and 44 are mental disorders and the other cases are often associated with mental disorders and research has shown that most mental disorders begin in childhood and in adolescence (2009). Poor mental health can have an effect on wider health and development of adolescents in associations with social outcomes such as higher alcohol, tobacco and illicit substances, pregnancy, school dropout and even the act of wrong behaviors of a child.
When comparing the studies in question there are some differences to note. Reavley et al. (2010) reports that the sample size was initially 14,805 prospective participants, but only 8841 completed the interview process. Comparatively the study conducted by Adamson et al. (2006) reports that the research sample was 105, which would suggest that the study done by Reavley et al. (2010) had more validity in regards to results. Furthermore Reavley et al. (2010) had a selection bias in their participants with people aged between 16-24 being the targeted group as this population was who the study was aimed at. Both studies used the World Mental Health Composite International Diagnostic Interview along with the DSM IV for their survey instrument. The study conducted by Reavley et al. (2010) was directed by Mental Health Services compared to the study conducted by Adamson et al. (2006), which was run through a particular alcohol and other drug service. When looking at the study in quest...

In practice, there are other assessment tools such as….which I could have used but the CPA is a recommended National Standard Framework for Mental Health, introduced in 1991, to supply a framework for effective mental health care (DOH 1999; DOH 2008) and to safeguard all service users (SU) by appropriate assessment and review (Gamble, 2005). But it is time consuming, in practice and paperwork. Service users’ participation has been repeatedly disappointing; studies constantly report little awareness of the CPA policy (CPAA, 2006; McDermott, 1998).
An assortment of quality-of-care problems must be addressed in order to improve health conclusions and contain costs. And the need to improve conclusions for individuals with mental illnesses, predominantly the severe cases, is clear (Bazelon Center for Mental Health Law, 2015). My experience in mental health clinical was very different from any other clinical I had before. In a mental health clinical setting, I am not only treating client’s mental illnesses, I am also treating their medical problems such as COPD, diabetes, chronic renal failure, etc. Therefore, it is important to prepare for the unexpected events. In this mental health clinical, I learned that the importance of checking on my clients and making sure that they are doing fine by performing a quick head-to toes assessment at the beginning of my shift. I had also learned that client’s mental health illness had a huge impact on their current medical illness.
