Karmen is a 50-year-old married who told her psychiatrist that she was considering suicide through overdosing on Advil. She complains of severe back pain that has left her with a “poor mood”. She talked about the injury for a long period of time. When doctors did not validate her injury, she described feeling abandoned. Karmen had gained weight and was upset about that. She did not take making suicidal comments seriously and often just used them as a threat towards her husband. She craved the attention of the doctors, and was flirtatious with the person who interviewed her. Karmen’s husband said that she talked about suicide on a regular basis. This would assume that her drive for attention is marked by current feelings of worthlessness, and that worthlessness is caused by depression. It would also assume that her suicide threats are the result of actual suicidal feelings. I do not believe that her threats are credible at this point. For these reasons, I narrowed down the possible diagnoses, and eliminated major depressive disorder.
The last and most accurate is histrionic personality disorder (p. 667). Karmen meets the following criteria: uncomfortable when not the center of attention, “inappropriate sexually seductive or provocative behavior” (American Psychological Association, p. 667), shallow emotional expression, attempting to gain appearance through physical appearance, self-dramatization, and considers relationships to be more intimate than they really are. We see this when Karmen begins calling the doctors around her by their first names.
For Karmen, we can consider couples’ counseling and individual counseling to help with the suicidal thoughts and depression. Karmen can meet with a medical provider to get a second opinion regarding her back injury. We can also work with Karmen to practice mindfulness and increased communication skills.
