The topics of health and safety are a hospital’s main concern when it comes to treating their patients. A hospital’s main goal is the patient; without patients, there would be no one for physicians to treat, therefore making it hard for hospitals to be run. Health care managers and leaders provide aid to patients by listening and understanding their patient’s needs as well as concerns. These issues are addressed within healthcare facilities in order to improve their system which can include making changes within the hospital in order to fulfill the patients’ needs to create a better environment overall. This is a patient centered operation with innovation being a key target. This allows you to fix issues in all aspects of your system and get a more efficient outcome. “Systems thinking can help identify unproductive interactions and eliminate these adverse events as well as reinforce innovative and efficient processes.” When hospital leaders use this approach of thinking, they can tackle every issue in all parts of their healthcare system, and are not focused on the issues they are accountable for. This approach could also be beneficiary in advancing team and staff communication and work ethics. Systems thinking not only increases the likeliness of better solutions and insights, but it is an effective mechanism that routinely had successful
SGH has been plagued with patient quality issues, therefore SGH finds itself in a situation which is inherently antithetical to the mission of the hospital. The costs of healthcare continue to rise at an alarming rate, and hospital boards are experiencing increased scrutiny in their ability, and role, in ensuring patient quality (Millar, Freeman, & Mannion, 2015). Many internal actors are involved in patient quality, from the physicians, nurses, pharmacists and IT administrators, creating a complex internal system. When IT projects, such as the CPOE initiative fail, the project team members, and the organization as a whole, may experience negative emotions that impede the ability to learn from the experience (Shepherd, Patzelt, & Wolfe, 2011). The SGH executive management team must refocus the organization on the primary goal of patient

If I was to become the CEO of a large health care organization, I would investigate and analyze all the information to determine what needs to be improved within the organization in order to make the best decision for the company. There are three major elements of quality: structure, process, and outcome”(Burns, Bradley, & Weiner, 2011, pg 251). One way to improve the quality of care in my organization is to be passionate and excited about the engagement of consumers. The patients need to be able to have access to the right information to educate themselves about their health care decisions. If they are active working with the physicians it can reduce emergency hospital visits and improve treatment and quality of life that is associated with different chronic diseases (Aulbach, 2015). As for my staff, I would ensure that they have all the equipment as well as the

Vital improvement for patient safety has triggered an enormous amount of positive change in the healthcare system. There were “1.6 million adverse events each year that led to 180,000 deaths” (Liang & Mackey, 2011). In a review, avoidable errors led to $19.5 billion dollars in healthcare expenses (Liang & Mackey, 2011). The National Patient Safety Agency analyzed 425 deaths from acute care hospitals and found “15% of the deaths were related to unrecognized patient deterioration” (Higgins, Maries-Tillot, Quinton, & Richmond, 2008). This finding led to the Institute for Health Care Improvement’s promotion for the use of an early warning scoring system to assist with identifying deteriorating patients (Albert & Huesman, 2011).
Each year this panel of experts put a microscope on patient safety across the board. They decide where upmost attention needs to be paid. Sometimes items leave the list because there are been strides take to improve in that area and sometimes it continues to stay on the list because they believe the relevance and importance is growing. A hospital is a difficult place to run because there so many aspects to manage. There are many types of doctors and nurses, and so many departments in this type of facility. The patients come in a wide variety of different ailments, needs, colors, sizes, personalities, and beliefs. Not to mention, with all of the equipment, devices, and people coming and going a hospital can seem like a small town in itself. That is why it will take a group effort, open communication, and positive reinforcement to keep it running smoothly. I will address this case study by identifying each problem, advise an adequate solution for each problem, and give a reason for each solution.
It is right of a patient to be safe at health care organization. Patient comes to the hospital for the treatment not to get another disease. Patient safety is the most important issue for health care organizations. Patient safety events cost of thousands of deaths and millions of dollars an-nually. Nurses play a critical role in ensuring patient safety during direct care delivery. They interact with physicians, pharmacists, families, and other healthcare team members at the bedside consistently. Nurses are essential in promptly coordinating and updating team members on the patient's condition. To ensure high-quality care, nurses are responsible for various tasks such as monitoring patients for clinical deterioration, identifying errors and near-misses, understanding care protocols and system flaws, and recognizing and communicating changes in patient conditions. The vigilance of nurses at the bedside is crucial to patient safety.

Institutes can improve the efficiency of patient care that is rendered by using this system theory. Moreover, the nursing staff will feel more assured and contented coming to work, which will lead to increased employee satisfaction and retention. References Hayajneh, Y. (2007). The purpose of his article was to find a better way to prevent healthcare-associated infections (HCAI) and explain what could be done to make healthcare facilities safer. The main problem that Cole presented was a combination of crowded hospitals that are understaffed with bed management problems and inadequate isolation facilities, which should not be happening in this day and age (Cole, 2011). He explained the “safety culture properties” (Cole, 2011) that are associated with preventing infection in healthcare; these include justness, leadership, teamwork, evidence based practice, communication, patient centeredness, and learning. If a healthcare facility is not honest about their work and does not work together, the patient is much more likely to get injured or sick while in the

The introduction of innovation to healthcare lead to a systems thinking. Systems thinking required the healthcare industry to re evaluate the the structural components of healthcare and how to improve it. This change lead to change management where leaders implemented the systems thinking to sustain innovation occurring in healthcare.
