When McMurphy decides to stand up to Nurse Ratched, there is “no fog” (130). Kesey’s metaphor of the fog represents the haze of inaction that hovers over the patients of the ward. With the oppressive Nurse Ratched in charge, the patients are not able to stand up for themselves and are forced to be “sly” to avoid her vicious punishments (166). When the patients avoid confrontation with the Nurse, they are guaranteed safety by hiding in the fog, complaisant with their standing. Kesey shows the reader the world within the asylum of Portland Oregon and all the relationships and social standings that happen within it. The three major characters’ groups, Nurse Ratched, the Black Boys, and McMurphy show how their level of power effects how they are treated in the asylum. Nurse Ratched is the head of the ward and controls everything that goes on in it, as she has the highest authority in the ward and sabotages the patients with her daily rules and rituals. These rituals include her servants, the Black Boys, doing anything she tells them to do with the patients.

The novel starts off with the arrival of the protagonist Randle McMurphy into a mental institution also known as the ward. Instantly McMurphy turns the ward upside down by disturbing the law and order set by the antagonist Nurse Ratched. McMurphy being the impulsive character he is makes a bet with his fellow patients in the ward and says that he will make Nurse Ratched lose her composure without facing any consequences. McMurphy is a man of no fear and does exactly that but soon realizes that Nurse Ratched also plays with fire. She fights back and enforces the law and order.
Each of the men who are admitted at the institution have some sort of ‘fault’ that makes them atypical. As a result of been made fun of for these characteristics, they hide away from society inside the mental institution to escape the pain of being individuals. Nurse Ratched's most threatening instrument is preying on the group mentality of the patients in order to generate fear and self-hatred. Her steadfast rule is that patients are not allowed to be by themselves and must stay groups at all times. This is because she is aware that only in the privacy of their own room can they evaluate and develop the strength of will and character to go against her authority. The way in which the Nurse gives out orders is commanding in a sense that the tone and reaction from the patients give the audience the sense of impending doom. The one character who does show a strong sense of individualism and disobedience against Nurse Ratched, McMurphy, ends up getting destroyed. The context behind this theme is very significant in the fact that it involves a major cultural phenomenon of the 1950s- the Beat Generation.
