Introduction:
Major challenges face today's health care system is maintaining safety in health centers(1). Understanding work environmental factors that need improvement such as safety climate and team- work could help in maintaining a satisfying hospital environment and, as a result, might retain the qualified nurses and enhance their intent to stay levels. Safety climate is defined as efforts to minimize risk of harm to patients and providers through both system effectiveness and individual performance (2). In a survey of 179 ICUs representing 7846 staff members in 3 countries(United States, United Kingdom, and New Zealand),Sexton et al(6). found significant variation among clinical areas in responses to 6 safety factors: team work climate, safety climate, perceptions of management, job satisfaction, working conditions, and stress recognition. These findings were supported by a more recent study (7) of 30 ICUs in the United States. Also, Huang et al(8) reported varied perceptions between ICUs within a single health care facility in the United States and noted that ICU nursing directors tended to overestimate the attitudes of staff members, particularly for teamwork. Significant variations in attitudes between nurses and physicians(9,10) and between different levels of staff(ie, frontline staff and management staff) have also been reported, particularly in the organizational culture factors of perceived working conditions and teamwork. Differences between nurses and physicians have also been reported in other clinical areas, such as the operating room(11).
Healthcare organizations have started to consider safety climate as a vital issue in the success of the organization, and there is a major interest in measuring the attitude and perception of healthcare providers toward safety climate (2). Therefrom first step to improve safety culture is to measure it and according to safety climate is an unsustainable concept and is influenced by native and specific conditions of the same place (8). Safety competency is essential for high-quality care in the medical field. Nurses play an important role in setting the bar for quality healthcare services through patient safety mediation and strategies. The QSEN definition of safety is that it “minimizes risk of harm to patients and providers through both system effectiveness and individual performance.” This papers primary purpose is to review and better understand the importance of safety knowledge, skills, and attitude within nursing education, nursing practice, and nursing research. It will provide essential information that links health care quality to overall patient safety.
According to an article in Health Services Research, safety is one of the main reasons that HCAHPS/Press Ganey surveys patients after their hospital stays (Isaac, Zaslavsky, Cleary & Landon, 2010). This positive aspect brought about by HCAHPS/Press Ganey surveys is the re-focus of patient safety, something that should be a top priority and nursing responsibility of all patient care. A direct example of this focus is that if a patient experiences a serious safety event or is harmed by a medical error, his or her overall experience will be negatively impacted. (Isaac et.al., 2010). Cohen (2015) predicts that if the focus of healthcare shifts to the delivery of safe, compassionate, high-quality care, the patient experience and satisfaction with their overall care is likely to rise. There is also evidence that increased patient satisfaction is important for improving patient adherence. How patients perceive the receptiveness of the unit’s hospital staff likely reflects the hospital 's safety culture thus promoting adherence to treatment guidelines (Isaac, Zaslavsky, Cleary & Landon, 2010). Patients are more likely to continue suggested healthy habits and be compliant with their medication if they are satisfied with their healthcare practitioners. Patient safety is always the primary goal any hospital, nursing unit, and health care team. As the main caregiver, a nurse’s responsibility is to keep the patient safe, and free from injuries while the patient is hospitalized (Smith, 2012). The definition of nursing competency lack a direct universal definition, and technique and ways nursing supervisor and managers can evaluate patient care that was provided by a novice nurses (Smith, 2012). Integrating safety into the nursing practice, education and research is paramount to the effectiveness of the profession in so many ways as will be discussed in this paper. Patient’s safety will be compromised because increase of patient to nurse ratio will lead to mistakes in delivering quality care. In 2007, the Agency for Healthcare Research and Quality (AHRQ) conducted a metanalysis and found that “shortage of registered nurses, in combination with increased workload, poses a potential threat to the quality of care… increases in registered nurse staffing was associated with a reduction in hospital-related mortality and failure to rescue as well as reduced length of stay.” Intense workload, stress, and dissatisfaction in one’s profession can lead to health problems. Researchers found that maintaining and improving a healthy work environment will facilitate safety, quality healthcare and promote a desirable professional avenue.

The sources used were retrieved from the online Shapiro Library. Electronic databases, including CINAHL were used to do a multi-search for sources. Key works used included nursing safe staffing and nursing staff ratios and patient safety. The criteria used to select the articles were peer reviewed articles within the last five years.
In health care, safety does not only pertain to the patient, but to all of the staff as well. Although this is important, it is critical that nurses are safe, since they usually interact with the patients the most. If a nurse does not follow correct safety and health practices, they may cause harm to the patient, which may end up in a lawsuit if the damage is bad enough. If safety measures are followed and nothing wrong happens, this saves the facility money and it could possibly gain money if the patients refer the facility to other potential customers.
