Although the budgetary setting is limited, there are always increasing pressures and demands on the public health system (Sten, John, Rick & Lyne, 2006). Hospitalisation rates are increasing marginally due to many health conditions, and it is expected that this rate is repeatedly increase over the next 20 years. Even though there are number of causes such as wide incidence of chronic disease, ageing of the population is considered to be major issue for the future health system pressure (Queensland Government, 2010). Hence, many developing countries currently struggle to meet the needs and care of the predicted ageing population (Sciegaj & Behr, 2010). In this context, it is essential that services are well planned, with the capacity to react properly to advancing changes in order to meet community needs. The purpose of this paper is to discuss how Electronic Medical Records (EMR), affects healthcare delivery. I will discuss the positives and negatives this issue has on healthcare and how it effects the cost and quality for healthcare services. In addition, I will identify any potential trade-offs to cost or quality. Lastly, I will discuss how the EMR affects my job as well as any challenges or opportunities this issue presents.

In conclusion, we can agree from the above information that quality of life is significant in each of the goals discussed. However, the Healthy People 2020 goals and objectives must serve as a guide to boost national health promotion and disease prevention that will not only reduce disparities but also eliminate disparities. Both the federal and state governments, public and private health sectors will no doubt partner and focus in these objectives. It is, therefore, very imperative that necessary tools and strategic managements is used to focus on health equity to improve the health of all groups, create social and physical environment that promote good health, promote quality of life, healthy development and healthy behavior across life stages that will benefit all by the year 2020.
Often in practice, we as nurses deal with a variety of diseases and treatments and often have to react to the illness that the patient presents with upon our interaction. The prediction is that all health care providers will spend at least 50 percent of their time working with older adults; increased competency while eliminating ageist attitudes is paramount for quality health care (Ferrini & Ferrini, 2013, p. 15).
Formulation is the second stage after identifying the problem and setting the agenda. This stage involves the proposal of solutions to the issue. The policy makers propose several courses of actions and solutions to addressing the issue. Then come up with one effective solution that will solve most of the issues (Boundless, 2013).

Central to its design, the cyclical nature of PAR promotes ongoing observation, reflection and action to produce community-devised plans that will achieve sustainable, positive outcomes (Koch, & Kralik, 2008). Evaluation processes often involve matching objectives with outcomes and noting differences in the community before and after implementation. This may include surveys that measure the impact of research and implementation across different areas of the communities' health and wellbeing including health related behaviours (Billings, 2008). However, as it often takes time to see significant results after implementation, it is imperative that not only culturally appropriate and competent, designs and methods have been employed throughout to ensure greater chance of success in meeting community needs and objectives, but continual collaboration is employed for feedback to guide the PAR process (Koch & Kralik, 2008). If PAR is implemented correctly in respect of key considerations outlined within this plan, feedback loops would be generated from building and maintaining strong working relationships, and the facilitation of ongoing dialogue throughout all phases.
To achieve vision 2030, health sector is a key pillar through provision of accessible, quality and relevant health services to have a healthy workforce. Increasing allocation of resources, improvement in health personnel and facilities, better health management are among the strides made. However, challenges still exist due to infrastructural constraints, inadequate human resources, increasing cost of medical care, financial constraints, HIV/AIDS Pandemic, increasing non communicable diseases and high poverty levels.
