The vast majority of offenders who enter jails and prisons on a daily basis bring numerous problems with them that deal with health issues and they receive very little or inadequate treatment while locked up. Then when they return to communities these inmates face striking challenges in getting health care and public health services that can improve health and prevent disease. These problems are known to people on the health care scene, but nobody does anything. So, to improve this there needs to be a thorough review of the causes and costs due to limited access to good quality health care. By doing this it provides a framework for the development of wide-ranging strategies for the reintegration of returning offenders. In a recent study done by the American Journal of Public Health, they found that providing prisoners with adequate health care while behind bars and where they’re released to can help improve public health. Given that all these health issues impact reentry results and that nearly all returning prisoners have some type of health issues, an assessment of health requirements should be included in ever inmate’s reentry planning process. This would assess every returning prisoner’s eligibility for any form of public assistance which includes Medicaid or any disability needs. This would then begin the application or rebuilding process for prisoners being released. Every returning prisoner, even the ones that did not receive any health services while in prison, should be cultured on important health care resources that fit their needs, so they know what to do when they're released back into society. Unfortunately, this rush to develop and implement preventive measures has resulted in a degree of polarization which has hindered progress towards implementation of effective prevention measures. Prisons and jails offer uniquely important opportunities for improving disease control in the community by providing health care to disease prevention program to a large and concentrated population of individuals at high risk for disease. Inmates often have little interaction with the health care system before and after being incarcerated. (U.S. News & World Report) The bureau of Justice Statistics (BJS) reported that in 1999, HIV/AIDS in prisons and jails was a growing problem in American correctional facilities. Prisons and correctional facilities in the United States have changed from rehabilitating people to housing inmates and creating breeding grounds for more violence. Many local, state, and federal prisons and correctional facilities are becoming more and more overcrowded each year. If the Department of Corrections (DOC) wants to stop having repeat offenders and decrease the volume of inmates entering the criminal justice system, current regulations and programs need to undergo alteration. Actions pushed by attorneys and judges, in conjunction current prison life (including solitary confinement), have intertwined to result in mass incarceration. However, prisoner reentry programs haven’t fully impacted positively to help the inmate assimilate back into society. These alterations can help save the Department of Corrections (DOC) money, decrease the inmate population, and most of all, help rehabilitate them. After inmates are charged with a crime, they go through the judicial system (Due Process) and meet with the prosecutor to discuss sentencing.
While, equality of healthcare provision in America continues to be a leading topic of debate. Healthcare rights for incarcerated persons are largely absent from this national conversation. Healthcare affordability and accessibility to quality treatment, medicine and doctors remains a priority across all ages, races, genders and political parties. Because “the state” is given the power of autonomy from the provision of law under the federal government, it should be the responsibility of “the state” to provide adequate healthcare services and treatment to persons who are incarcerated.
When people think of reform movements, they often look for one key sign, and ask one key question of whether that the reform was a success. Did the reform create a lasting change in the way people view the institution that was reformed? All the great reformation movements, from Horace Mann and his education reforms, to Martin Luther, and the Protestant Reformation, to the civil rights movement, all created lasting change in the minds of the average person. One other reform, often overlooked historically is the Prison Reform movement. As the world shifted from 18th to 19th century ways of life, many key aspects of life underwent tremendous change. As the United States gained their independence from Britain and began to shape their own identity, the reforms and revolutions that occurred in this infantile stage of its history played an immeasurable impact on the future of the entire country, with the most notable and impact reform being the reformation of prisons from the 1820s until 1860.
The U.S is only 5% of the world’s population and houses a quarter of its prison inmates; well over 2 million people. In the past decade the war on drugs has filled many state and federal prisons with a numerous amount of inmates. Building new prisons is not the answer to tackling the prison overcrowding dilemma. The U.S doesn’t have the money due to economic strains, and it will not solve this issue head on as needed. “California may be forced to release up to 33,000 prisoners by 2013” (Shapiro & Wizner, 2011, p.1.). Some women and men do not belong in prison, and should be given other opportunities to sought help. Prison overcrowding is a growing concern in the U.S today. There are many different alternatives to end prison overcrowding versus releasing them into the community. For example by launching a parole support group or treatment or rehabilitation programs for inmates as well as ex-offenders, house arrest or probation are other routes to explore.
The number of Americans that are in prison has elevated to levels that have never been seen before. Prisons in the US have always been crowded ever since the first prison was invented (Jacobs and Angelos 101). The first prison in the US was the Walnut Street Jail that was built in Philadelphia in 1773, and later closed in the 1830’s due to overcrowding and dirty conditions (Jacobs and Angelos 101). Living in a prison for a long time becomes difficult for all inmates especially those who are mentally ill face stress when their environment suddenly becomes bars, harsh lights, and super maximum strict schedules. The inmates are forced to face the strict policies and conditions of custody in order to survive in the prison. These prolonged adaptations to the hardship and frustrations of life inside prison lead to certain psychological changes. It is undeniable that mass incarceration devastates families, and disproportionately affects those which are poor. When examining the crimes that bring individuals into the prison system, it is clear that there is often a pre-existing pattern of hardship, addiction, or mental illness in offenders’ lives. The children of the incarcerated are then victimized by the removal of those who care for them and a system which plants more obstacles than imaginable on the path to responsible rehabilitation. Sometimes, those returned to the community are “worse off” after a period of confinement than when they entered. For county jails, the problem of cost and recidivism are exacerbated by budgetary constraints and various state mandates. Due to the inability of incarceration to satisfy long-term criminal justice objectives and the very high expenditures associated with the sanction, policy makers at various levels of government have sought to identify appropriate alternatives(Luna-Firebaugh, 2003, p.51-66).
The “Tough on Crime” and “War on Drugs” policies of the 1970s – 1980s have caused an over populated prison system where incarceration is policy and assistance for prevention was placed on the back burner. As of 2005, a little fewer than 2,000 prisoners are being released every day. These individuals have not gone through treatment or been properly assisted in reentering society. This has caused individuals to reenter the prison system after only a year of being release and this problem will not go away, but will get worst if current thinking does not change. This change must be bigger than putting in place some under funded programs that do not provide support. As the current cost of incarceration is around $30,000 a year per inmate, change to the system/procedure must prevent recidivism and the current problem of over-crowed prisons.
An estimated 650,000 offenders are released from prisons each year. Most generally leave with only a few dollars, some clothes, and possibly a bus ticket. Release practices like this are common and can be especially disastrous for mentally ill inmates. If immediately released without access to health care, the mentally ill will suffer from interruption of continuity of care. In prison, they may have been receiving medication, therapy, or other forms of treatment. Interruption of care could lead to excelled deterioration in their mental health.
