There has been some advancement in the adaptation of EBP as the standard of care in recent years, however there is still much work to be done for the standard to be used daily in practice by point of care providers. An increased effort is required to provide the tools that point of care clinicians needs to use the best evidence from research and their practice to improve their healthcare system, practitioner and patient outcomes.
The implementation of clinical care that is steeped in tradition or based upon outdated policy results in a multitude of improved health, safety, and cost outcomes, including a decrease in patient morbidity and mortality. The level of evidence and the quality of evidence leads to a convincing evidence which provides clinician the confidence that is needed to change clinical practice.
The best treatment for the patients and clients is the goal for evidence-based practice. However, the application of the evidence in healthcare will be effective only if applied to practice. The relevance of the evidence to the practice of healthcare can lessen inaccuracies in providing care. As estimated from the center for Disease Control and Prevention, 25.8 million is people in the United States have either diagnosed or undiagnosed with diabetes. The improvement in the strategies to address the management of the disease and its complications is essential to the medical and financial issues of the disease.
Self-management of the diabetes will support pre-diabetic and individuals diagnosed with diabetes. The support from different members of the health care tea and the community that will help in implementing the research.
The case study approach as a single descriptive design will be used in this study. The research will focus on a group or population with pre-diabetes and patients diagnosed with diabetes. The study will be unique, specific and bounded in nature to describe information and analyze the case.
The qualitative research will be use in this study to constitute effective practice of the findings. The characteristics of qualitative research as a unique, thus, describes outcomes of the work intervention and provides hypotheses. Diabetes has recently become a focal point of health care systems around the world due to its high prevalence and the severity of secondary complications caused by the disease. Over the course of my project on diabetes, I have had the opportunity to speak with a group of diabetics to understand from a patient’s perspective how diabetes is managed in a rural community. While I found that while some patients ignored treatment and refused to make any dietary changes, the majority of the patients I interviewed were well-informed and actively managing diabetes in their everyday life.

Sackett (1996) explains about evidence based medicine (EBM) and that by using the best clinical expertise alongside the best available external clinical evidence from systematic research then the patient would receive the best treatment available. When caring for patients it is fundamentally important to have a good selection of up to date evidence Based Practice clinical articles to support research strategies, this allows professionals to assemble the most resent and accurate information known which enables them to make decisions tailored to the individual’s plan of care. It is essential to have clinical expertise and have the involvement from the individual patient, they must have full engagement and incorporation in order to have the accurate evaluation.
The purpose of this study is to research the disease Type 2 Diabetes and to discover whether it is being effectively treated and prevented in Merced County, California. According to the National Institutes of Health, Diabetes Mellitus is “a lifelong (chronic) disease in which there are high levels of sugar (glucose) in the blood. It is also the most common form of Diabetes”.1 Furthermore, there are many risk and lifestyle factors associated with this disease, but the most prevalent are; obesity (#1 risk factor), sedentary lifestyle, unhealthy eating habits, family history and genetics, increased age, high blood pressure and high cholesterol, and a history of gestational diabetes.2 The long term complications of having type 2 diabetes can include but are not limited to; eye problems (cataracts and glaucoma), foot problems (neuropathy/ nerve damage), skin problems (infections), high blood pressure (which raises your risk for heart attack, stroke, eye problems and kidney disease), hearing loss, oral health, mental health and early death.3
“Evidence-based practice is the integration of best research evidence with clinical expertise and patient values” (ASHA). Staff of ASHA embarked upon the new idea of Evidence-Based Practice (EBP) in 2005. The goal of EBP is the integration of: clinical expertise/expert opinion, external scientific evidence, and client/patient/caregiver perspective to provide high-quality services reflecting the interests, values, needs, and choices of the individuals we serve (ASHA). The four steps of EBP include: framing the clinical question, finding the evidence to suppose the question, assessing the evidence, and making the clinical decision. Researchers have studied the status of current implementation of EBP in the field, challenges to implement EBP, the debate of adopting EBP framework in our field, and the use of EBP in SLP practice and EBP knowledge.
EBP incorporates an organized search for the best up to date empirical research, with clinical expertise, to answer a focused clinical question concerning a patient or a quality improvement issue in the clinical environment. The focused question addresses the patient population, area of interest, comparison intervention or control group, and desired outcome (PICO). Using this question style saves the nurse time and will yield the best available relevant evidence.
