The organization will be owned and managed by Belinda Gardner, MSW as a sole proprietorship. She will be responsible for ensuring the behavioral healthcare of her clients and creating a viable and profitable behavioral outpatient health organization. Gardner will focus on assessing, diagnosing, and treating depression, anxiety and substance abuse of adults eighteen years and older. She understands that there are many factors that can affect depression, anxiety and substance abuse, including family, environment, and medical conditions. Ms. Gardner will ensure that each client understands both risk and benefits of the services being rendered to them upon the obtaining of their written consent. She will refer clients when appropriate, to intense treatment facilities and/or to hospitals for further treatment when it is outside of her scope of practice. The organization will utilize new equipment and a trained staff that will be able to optimize the care of each client. Staff consists of three employees: a licensed clinical social worker, mental health tech and an administrative
assistant. The organization will be dependent upon a one-time start-up cost estimated at $45,000 in the beginning. This amount of money will be used to purchase office equipment, supplies, furniture, stationery, and other start-up expenses. The organization will also need about $15,000 for cash on hand at the start-up. Expenses and initial cash requirements will be funded by First Tennessee Bank in Memphis, Tennessee. Five Star Outpatient Behavioral Health long-term assets consist of $175,000 for the purchase of the building. Ms. Gardner will finance the purchase of a 30-year loan that is guaranteed by her personal assets. The major revenue drivers will come from federal and state grant funding, third-party payers such as Medicaid and private insurance company as client volume increases. The revenue which includes cost of services sold consisting of labor, materials, etc. gross margin will be approximately $85,000 the first year according to the funders policies and guidelines. Expenses are estimated at approximately $45,000 the first year which consists of payroll, marketing/promotion, and office supplies. The projection of the organizations for the first five years are based upon increased client volume. We will begin supporting our expenses from revenues and obtaining a positive net profit according to the reflection in the chart below. Formulation of Problem/Needs: The client 's presenting problems are caused by her mother’s emotional verbal abuse. In spite of all, her emotional problems Ana maintains a positive outlook towards her future. Ana demonstrates self-determination as she clearly expresses her current issues. She struggles with overeating because she feels unloved and worthless. Ana is seeking services to overcome the resentment she feels towards her mother. She is requesting help to manage her coping skills and reduce her feelings of depression. According to Ana these feelings started at a young age. Ana’s current challenges are learning to cope with her mother’s verbal abuse. Anna will arrange monthly meetings with her social worker to talk about what methods she’s used to coping with her depression. Assessment is about collecting information on a person’s needs and circumstances, and making sense of that information in order to identify needs and decide on what support or treatment to offer (DOH, 2002). There are many tools for assessing mental health needs of patients with depression. Beck Depression Inventory (BDI) (Beck et. al, 1961; 1988), Beck Anxiety Inventory (Beck et al, 1993) and Zung Self-Rating Depression Scale (ZSDS)(1965) among others. These tools tell me how someone is feeling. This information is important for the CPA.

As a Gestalt practitioner, I would take the client’s history and gather impressions of their environmental field; the role peers have played in the process, and the emotional factors. The site for Meridian is located in North Central Florida, the building is very clean, and the waiting area was well stock with materials that gave insight to the different types of services you could receive through the programs they offered.

The Spectrum Behavioral Health Staff is consistently committing significant mistakes. That is because of misleading information are constantly given to them. This situation is causing frustration and low levels of job satisfaction among staff members. As a consequence, more and more employees have been resigning from their positions. I have observed, that the company policies and general operations are significantly inconsistent, making it challenging for senior employees to put into practice what they have learned during their training process. I have also observed that new hires are usually trained by multiple senior employees during busy office hours, which in fact can be causing misleading information and confusion. These problems
The diagnosis of this type of substance use disorder often is difficult because the symptoms of substance abuse can be comparable to the symptoms of other medical and behavioral problems that are found in older adults, such as dementia, diabetes, and depression. Many healthcare providers underestimate the extent of substance abuse problems among older generation, therefore, do not screen older adults. Seniors often live with or are supported by their adult children or other family members because of financial necessity and with their substance abuse it is placing additional financial hardship on supporting families, as well as the psychological damage. If the older adult’s spouse is present, they are likely to be an older adult as well, and may be perplexed by their partners new and disruptive behaviors and may not be able to understand the addiction. Therefore, their spouse may not be in a position to help assist in recovery. Their children may take on a parental, caretaking role. This role reversal can be stressful, painful, and embarrassing. In some cases, grown children may stop providing financial support, physical abuse, and assert emotional control because it is the only influence they have over the parent. Children may cut ties with the parent due to their substance abuse. Cutting ties have only increased the parent’s isolation and may worsen the predicament.
