There are many best practices that are currently in use throughout the country to reduce readmissions to hospitals. However, the best evidence for my proposal is based on guidelines from the Center for Medicare and Medicaid Services (CMS) which focus and initiatives are to reduce excessive readmissions and encourage health care providers to implement interventions to improve the health of their patients while decreasing costs (Adeoye & Pineo, 2014). CMS uses the Interact 4, SBARs and Care Paths to help nurses communicate change in status of residents to physician, or Nurse Practitioner (NP). Moreover, many skilled centers are implementing a program to employ ARNPs who can assess patients within twenty- four hours of admissions, review discharge medications, and order diagnostic exams to analyze patient’s condition, and to implement interventions that would reduce the risk of readmissions within thirty days. This approach for reducing avoidable readmissions has been very effective when there is a working collaboration between the physician and the NP (Toles, 2012). Another approach that can help to reduce readmissions is to implement strategies to improve communication and education of staff. According to Goins, (2012), long- term care is changing; staff needs to change as well because higher acuity necessitates higher education and communication skills from the staff. Fortunately, CMS noted the change in long- term care, consequently, they implemented the Interact tools to improve prompt recognition of change in status, assessment, and communication of changes in condition efficiently to the physician (Goins, 2012). Interprofessional teams in health care are considered to be one of the best approaches to improve patient outcomes. Interprofessional teams provide the means to integrate patient care with input from many different professional disciplines (Rose, 2011). Nurses are an important part of the interprofessional team, since they are often the team member that is closest to the patient (Miers & Pollard, 2009). I recently participated in a team that developed a work flow for daily readmission rounds. The team was interprofessional, the hospitalist, who was an APRN led the team. There was the case manager and the primary nurse who were both RN’s. The team also consisted of a resident, pharmacist, nutritionist, physical therapist, and social worker.
Monitoring staff levels is an important factor. Also leveling the flow of patients in and out institutions could help to reduce wide fluctuations in occupancy rates and prevent surges in patient visits that lead to overcrowding, poor handoffs, and delays in care. Studies show that overcrowding in areas such as the emergency rooms lead to adverse outcomes, because physicians and nurses having less time to focus on individual patients. One study found that for each additional patient with heart failure, pneumonia, or myocardial infarction assigned to a nurse, the odds of readmission increased between 6 percent and 9 percent (Hostetter and Klein, 2013). All of which costs the hospital money.
Hospital readmission can impact the patient, nursing practice, the hospital, and the health care system. The patient’s quality of life can be altered physically, psychologically, and economically (Whittaker, 2014) and recurrent hospitalization is a good predictor of increased risk of mortality (Hummel, Katrapati, Gillespie, DeFranco, & Koellig, 2013). There is a shortage of all health care professions throughout the United States. One shortage in particular that society should be very concerned about is the shortage of Registered Nurses. Registered Nurses make up the single largest healthcare profession in the United States. A registered nurse is a vital healthcare professional that has earned a two or four year degree and has the upper-most responsibility in providing direct patient care and staff management in a hospital or other treatment facilities (Registered Nurse (RN) Degree and Career Overview., 2009). This shortage issue is imperative because RN's affect everyone sometime in their lifetime. Nurses serve groups, families and individuals to foster health and prevent disease.

Technological advances enable nurses to provide accurate, timely care for a patient. This is due to the fact that these advances enable doctors and nurses to quickly diagnose, explain and predict the health-illness status of a patient, thus allowing health care professionals to spend less time finding answers, and more time providing quality care. For nurses, this includes spending time with the patient establishing rapport, communication and a trusting relationship for optimum clinical care.
Following the QSEN model, this problem is a concern that falls under the safety category. I believe this can only benefit the hospital and patient care, and have a new way that the patient is cared for. Treating the whole family, instead of just the patient is what the future is all about. Implementation of this type of care requires creating a partnership between the patient, physicians, nurses, and patient’s families. This can only improve performance improvement, and treat the patient the way we would want to be treated. Nurse staffing is a complex issue. There are many factors which need to be considered when staffing a nursing unit. Evidence shows an association between Registered Nurses (RNs) having decreased workloads and better patient outcomes, including a decrease in patient mortality (Aiken et al., 2011; Needleman et al, 2011; How Lin, 2013; Patrician et al., 2011; Wiltse-Nicely, Sloane, Aiken, 2013).
