Ethical Analysis of a Nursing Case Study
`Ethics' is defined as ."..the basis on which people...decide that certain actions are right or wrong and whether one ought to do something or has a right to something"(Rumbold, 1986). In relating `ethics' to nursing care, "Nursing decisions affect people... nurses have the power to good or harm to their patients" (Bandman et al, 2002). In this essay, the author will also identify the most important ethical principles and concepts of Evan's case, will outline the different stages of one's approach to ethical decision-making by utilising the "DECIDE Model for Ethical Decision-Making" founded by Thompson et al (2000) and will make a decision on the best course of action to take as a nurse in this situation.
In "Defining the Problem", the first stage of one's approach to ethical decision-making, the key facts are that Evan is a married man committed to a psychiatric unit who contracted HIV after a homosexual affair without the knowledge of his wife, Claire. He refuses that his wife and family be told of his illness. The stakeholders involved directly are: Evan, who does not want his family to find out about his illness or affair; his nurse, who is under increased strain worrying about the burden of, perhaps, having to break the confidentiality entrusted in him; Claire, is the most direct stakeholder as she may have contracted HIV; Evan's partner during the affair is also at risk of contracting HIV and his subsequent partners are also at risk; the other members of staff, are in constant worry over Evan which means that they cannot concentrate fully on their other patients; Evan's family is also involved directly as they are worried about his well-being. Everyone comes across difficulties in their everyday life. It doesn’t matter how small you are or how big you are or even how tough you are, you run into a problem every single day. Some problems are easier to handle and you can work them out by your self like Beowulf did with Grendel. Some problems might be too difficult to handle or no way of concurring it by yourself. There are a lot of hard things we come across that we may need help on just like the dragon in Beowulf. Beowulf needed a little help with the dragon. The dragon was too much for him to handle and it was beating Beowulf. I have faced a few “personal dragons” on my own including my parents getting a divorce when I was young and collage.
One cannot fake being a nurse, one must be extremely genuine in order to perfect being a nurse; therefore, explaining why nurses enforce and value their code of ethics. The purpose of the code of ethics is to ensure patient safety and implement standard of care by following the nine provisions of ethics. The nine provisions explain the nurses’ responsibility while caring for a patient; for example, maintaining the rights and autonomy of a patient. Another point that the provisions highlight is being the patient advocate, nurses are in the front line of patient care and they must protect their patients. An important guideline that the nine provision emphasize is the need and requirement for nurses to continue with their education to promote beneficent and to avoid maleficent. The National Nursing Association (ANA) states that the nursing code of ethics “reiterates the fundamental and the commitment of the nurse” (Lachman, Swanson, & Windland-brown, 2015). The purpose of this paper is to highlight the obligations and duty of a nurse and why it is important when attempting to maintain standard of care.

Ethics has been a popular topic in nursing for a long time. Nurses are expected to demonstrate ethical decision-making as well as professionalism. I believe that in order to accomplish this, they need to use the ANA Code of Ethics as framework for their decision-making. It is also imperative for nurses to have a strong understanding of ethics, because they will be faced with many difficult ethical decisions that do not always have a straightforward solution.
This paper explores the legal, ethical and moral issues of three healthcare colleagues by applying the D-E-C-I-D-E model as a foundation of decision making as found in Thompson, Melia, and Boyd (2006). Issues explored will be those of the actions of registered nurse (RN) John, his fiancé and also registered nurse (RN) Jane and the Director of Nursing (DON) Ms Day. Specific areas for discussion include the five moral frameworks, autonomy, beneficence, Non – maleficence, justice and veracity in relation with each person involved as supported by Arnold and Boggs (2013) and McPherson (2011). An identification and review of the breached code of ethics and the breached code of conduct in reference with the Nursing, Council, and Federation (2008) will be addressed. Lastly a brief discussion on how the three schools of thought deontology, teleology and virtue had effects on each colleague (McPherson, 2011) .
This paper will focus on two BNUR leaner outcomes (University of Calgary, 2013) relevant to my learned understanding of nurses as ethical decision makers. I will outline the ways that I have seen ethics used and not used in practice, what I learned about ethics and its use in my theory courses and throughout my practicums, and I will reflect on how this understanding will translate into my professional practice moving forward.
Since ethical dilemmas are not always easily answered through the use of The Code of Ethics, ethical decision-making models are effective tools that can assist nurses in dealing with ethical issues. Ethical decision-making models provide a framework for working through difficult choices. They seek to define the limits of what is morally acceptable and help clarify the guidelines for making those difficult decisions. Ethical decision making models assist nurses in analyzing situations by focusing on understanding the patient needs, need for responsiveness to circumstantial considerations, and recognizing the uniqueness of each situation.

An ethical dilemma is defined as a mental state when the nurse has to make a choice between the options and choices that he or she has at her disposal. The choice is a crucial task as the opting of the step will subsequently determine the health status of the concerned patient, hence it requires a great deal of wisdom along with proper medical and health training before any such step is opted as it is a matter of life and death. Strong emphasis should therefore be on the acquisition of proper knowledge and skills so that nurses do posses the autonomy to interact with patients regarding ethical issues involved in health care affairs and address them efficiently. It is normally argued that nurses are not provided sufficient authority to consult and address their patients on a more communicative or interactive level as a result of which they are often trapped in predicaments where their treatments of action and their personal beliefs create a conflict with the health interests of the patient. Deontology is an ethical theory concerned with duties and rights. The founder of deontological ethics was a German philosopher named Immanuel Kant. Kant’s deontological perspective implies people are sensitive to moral duties that require or prohibit certain behaviors, irrespective of the consequences (Tanner, Medin, & Iliev, 2008). The main focus of deontology is duty: deontology is derived from the Greek word deon, meaning duty. A duty is morally mandated action, for instance, the duty never to lie and always to keep your word. Based on Kant, even when individuals do not want to act on duty they are ethically obligated to do so (Rich, 2008).
