Organizations are complex systems with high expectations of adaptive techniques that are set to improve the quality of care. Each organization delivers care with a comprehensive approach for continuous improvement, which goes beyond the small changes. They promote quality Improvement, aggressively through a systematic approach. The look of primary care has changed over the years. What was once a solo doctor’s office is now a small specialty group and growing into even larger practices. With this evolution there’s a need to create and manage plans, foster technology changes in medicine, attitudes in the work force, and heightened consumer and purchaser expectations (Sollecito, Johnson, 2011). Better monitoring and assessment of performances and finding ways to implement activities is a key factor to improvement. It has been found that education has very little effect in behaviors and that alone cannot produce quality Improvement but it requires an organizational focus as well. There are important factors to consider in quality guidelines and assessment just to name a few, the presence of organized systems, commitment to change, priorities, resources, and advantages to new processes. The other aspect is multiple strategies and tools to handle quality improvement not just one to focus on but several. Leaders recognize improving clinical quality will have many benefits for both, residents and the organization. For example, it will benefit residents by improving satisfaction, reducing complications, and improving their quality of life. It will benefit NCH by reducing turnover, improving occupancy rates, attracting top talent, and enhance employee satisfaction (Advancing Excellence in America’s Nursing Homes, n.d.). Restructuring of this program in certain areas is important “payment reform, enhancing the training pipeline, transforming practice, and buttressing the primary care safety net” (Fiscella, 2011).
In the past few years there has been much debate over the Affordable Care Act and its effects on the healthcare industry in the United States. The Affordable Care Act (ACA) “Will ensure that all Americans have access to quality, affordable health care and will create the transformation within the health care system necessary to contain costs” (The patient protection and affordable care act detailed summary, n.a.). However, what these transformations are and how they will affect the healthcare system, specifically primary care physicians are uncertain. Primary care physicians are the cornerstones for patients in the health care system. They act as a liaison between families and specialist physicians. Primary care physicians provide a variety of patient care services that involve multiple skill sets. They are in charge of diagnosing the patient and managing the plan of care. As a result of the newly implemented Affordable Care Act and the current shortage of primary care physicians “The primary care doctor is a rapidly evolving species -- and in the future could become an endangered one” (Okie, 2012).
It is no secret that the current healthcare reform is a contentious matter that promises to transform the way Americans view an already complex healthcare system. The newly insured population is expected to increase by an estimated 32 million while facing an expected shortage of up to 44,000 primary care physicians within the next 12 years (Doherty, 2010). Amidst these already overwhelming challenges, healthcare systems are becoming increasingly scrutinized to identify ways to improve cost containment and patient access (Curits & Netten, 2007). “Growing awareness of the importance of health promotion and disease prevention, the increased complexity of community-based care, and the need to use scarce human healthcare resources, especially family physicians, far more efficiently and effectively, have resulted in increased emphasis on primary healthcare renewal.” (Bailey, Jones & Way, 2006, p. 381).

The VA and MTF felt they needed to make the change with emphasizing team-based care. The VA provides care to approximately 8 million beneficiaries with 5.2 of the beneficiaries receiving primary care services. Between 2009 and 2011 there has been 1,000 primary care practices converted to the PCHM model. During that time the VA experienced a 4% decrease in hospitalization. At one of the MTF’s in San Antonio the emergency room saw a 14 percent of reduction in use of emergency room and urgent care.
