Another social factor impacting nursing and healthcare is the amount of cultural differences we see today. Despite the fact that diversity is an ever increasing concept, minorities are still underrepresented in healthcare and this runs the risk of resulting in a lack of access to care, high poverty rates, and unemployment (Masters, 2017). With more minorities entering the healthcare force, these disparities should decrease as no group should be left with less access and greater risks. Along with the need for greater representation, nurses “must first understand their own culture and how this can affect others” (McCracken, 2014) in order to provide culturally safe care. One of the main regulatory factors that has a direct impact on nursing would be licensure. Licensure has been vital in order to maintain the quality control of the nursing education process since the beginning of the profession (Shoemaker, 2002). The examination and licensure procedure first began in 1904 and “sets standards and qualifications for the licensed practitioner and holds the power to punish those who violate the law” in order to guarantee the safety of the public (Shoemaker, 2002). This procedure has continued over time, however, changes are starting to be made to allow this process to be much more convenient. Instead of requiring nurses to go through the process of licensure for each state they wish to practice in, states have begun to adopt a multi-state license that would allow them to only have one license. This multi-state license allows for nurses to have greater mobility between state borders while still requiring “uniform licensure requirements so that all states can be confident the nurses practicing… have met a set of minimum requirements” (About the NLC), ensuring public safety. Approximately 100 years ago a Board of Nursing (BON) was established to help ensure the protection of society through rules and regulations of proper nursing practice. The goal of the BON is to provide “regulatory excellence for public health, safety and welfare” (National Council of State Board of Nursing [NCSBN], n.d., para. 1). The duties of the board is to implement the Nurses Practice Act, handle licensures, accredit nursing programs, develop policies, rules, and regulations and develop standard practice (NCSBN, n.d.). The Institute of Medicine (IOM) recommended in a 2010 report that eighty percent of registered nurses (RNs) hold a baccalaureate degree by 2020. The shortage of registered nurses (RNs) in the United States has been a cyclical topic dating back to the 1960s. Only recently have employers in certain regions of the nation stated a decline in the demand for RNs. Consequently, according to the American Association of Colleges of Nursing’s (American Association of Colleges of Nursing [AACN], 2014) report on 2012-2013 Enrollment and Graduations in Baccalaureate and Graduate Programs in Nursing, American nursing schools denied admission to 79,659 qualified applicants from baccalaureate and graduate nursing programs in 2012. The reported decrease in job availability and rejected admissions has left many individuals to question if the nursing shortage still exists. On the other hand, some experts project that the United States will be short more than one million RNs by 2020 (Dolan, 2011). Although some parts of the country are in less of a demand than others, it is undeniable that there is a national shortage of RNs.
Environmental justice can influence the population’s health. This environmental justice is relevant to nursing, because awareness brings changes and can save and improve many lives. When a person in a hospital or in a community setting is affected by a health problem, the entire community is at risk, knowing the population is lack of knowledge and have limited access to understand health care system. The purpose of this post is to describe cultural baggage, ethnocentrism, cultural imposition, prejudice, discrimination, and cultural congruence. I will also give an example of each term to help you understand the terminology related to nursing care. I will define cultural self-assessment and explain why it is valuable for nurses to understand what their own self-assessment means. Finally, I will describe the five steps to delivering culturally congruent nursing care and how I have applied these concepts to my nursing practice.
As a nurse strive to provide culturally sensitive care, they must recognize how their client's and their perceptions are similiar as well as different. Nurse enhance their ability to provide client-centered care by reflecting on how their beliefs and values impact the nurse-patient relationship. To provide appropriate patient care, the nurse must understand her/his culture and that of the nurse profession. Cultural biases can be particularly difficult to identify when the nurse and client are of a similar cultural backgroup. When we recognize and know a culture, we will know what is right for our patient, and thus may impose our own values on the client by assuming our values are their values. Characteristics can be as diverse as ethnic background, language spoken, gender status, physical appearance, race, and religion to name a few. Migration from various countries is creating a diverse population with different cultures and languages within the United States. Due to these cultural differences and lack of knowledge, disparities are increasing. Studies have shown that both language barriers and lack of cultural customs can hinder the services provided to the patient by the healthcare worker (Renzaho, Romios, Crock, & Sonderlund, 2013). This study provided a positive outcome when communication and cultural mutual understanding took place and patients had a more positive health outcome. It is very important that nurses are diversified in various cultures in order to better care for our patients. According to Mareno and Hart (2014), cultural competency has become one of the core values being taught in nursing programs. Their study showed that the perceived level of cultural awareness and skills among the nurses provided was low. Awareness and knowledge levels increased with higher education.
