Health care systems are constantly trying to employ the triple aim in their healthcare delivery. This includes improving patient experience, improving population health and reducing cost. Unfortunately, healthcare has taken a turn as gaps in primary and preventative care continue to grow and expenditures projected to reach 20.1% of the nation’s GDP (Keehan et. al, 2017). Care management may serve as a solution to this problem by using appropriate interventions to treat specific populations and ultimately reduce health risks and the cost of healthcare. Health homes focus on chronic conditions and behavioral health and encourages caregivers to communication to provide comprehensive care. ACOs focus on improving quality and decreasing cost and use a Care Management Impact Model to track the program's impact on cost and utilization. Similarly, MCOs focus on individual payer’s goals alongside managing cost, utilization and quality of care. Lastly, MSOs are focused on VBP and population incentives. Now that we have a fundamental understanding of the current state of Care Management, we will take a look at the gaps in care this newly developing strategy will help to address. Effective care management across the continuum of health is dependent upon interoperability of a team and communication, throughout the process of care delivery. These two areas, interoperability and communication, are where providers and patients are most likely to see a breakdown within the continuum of health care delivery. Care management will ensure that patients are given access to the information they need, and will empower patients to make better decisions in regards to their care. By building communication we are building trust, and ultimately increasing value. Furthermore, by more accurately identifying those populations with modifiable risks, care management will help to effectively reduce health disparities. Racial health disparities are associated with substantial annual economic losses nationally, including an estimated $35 billion in excess health care expenditures, $10 billion in illness-related lost productivity and nearly $200 billion in premature deaths (Aryanian, 2016). Studies have analyzed how African Americans deal with an enormous amount of disease, injury, death, and disability compared to other ethnic group, and whites, Utilization of health services by African Americans is less frequent than other ethnic groups in the country. This non utilization of services contributes to health disparities amongst African Americans in the United States. Current and past studies have shown that because of discrimination, medical mistrust, racial/ethnic background, and poor communication African Americans tend to not seek medical care unless they are in dire need or forced to seek professional care. African Americans would rather self –medicate than to trust a doctor who might show some type of discriminatory
Large disparities exist between minorities and the rest of Americans in major areas of health. Even though the overall health of the nation is improving, minorities suffer from certain diseases up to five times more than the rest of the nation. President Clinton has committed the nation to eliminating the disparities in six areas of health by the Year 2010, and the Department of Health and Human Services (HHS) will be jumping in on this huge battle. The six areas are: Infant Mortality, Cancer Screening and Management, Cardiovascular Disease, Diabetes, HIV Infection and AIDS, and Child and Adult Immunizations.
Racial disparities in The United States health care system are widespread and well documented. Social and economic inequalities between racial minorities and their white counter parts have lead to lower life expectancy rates, higher infant mortality rates, and overall poorer health for people of color. As the nation’s population continues to become increasingly diverse, these disparities are likely to grow if left unaddressed. The Affordable Care Act includes various provisions that specifically aim to reduce inequalities for racially and ethnically marginalized groups. These include provisions in the Senate bill and House bill that aim to expand coverage, boost outreach and education programs, establish standards for culturally and linguistically appropriate practices, and diversify the health care workforce. The ACA, while not a perfect solution for eliminating health disparities, serves as an important first step and an unprecedented opportunity to improve health equity in the United States.

Healthcare disparities are when there are inequalities or differences of the conditions of health and the quality of care that is received among specific groups of people such as African Americans, Caucasians, Asians, or Hispanics. Not only does it occur between racial and ethnic groups, health disparities can happen between males and females as well. Minorities have the worst healthcare outcomes, higher death rates, and are more prone to terminal diseases. For African American men and women, some of the most common health disparities are diabetes, cancer, hypertension, cardiovascular disease, and HIV infections. Some factors that can contribute to disparities are healthcare access, transportation, specialist referrals, and non-effective communication with patients. There is also much racism that still occurs today, which can be another reason African Americans may be mistreated with their healthcare. “Although both black and white patients tended not to endorse the existence of racism in the medical system, African Americans patients were more likely to perceive racism” (Laveist, Nickerson, Bowie, 2000). Over the years, the health care system has made improvements but some Americans, such as African Americans, are still being treating unequally when wanting the same care they desire as everyone else.
The NHQDR 2012 is a comprehensive report that implies there are changes that need to occur at multiple levels within the health care system and public policy. The report implies that the health care system needs to become more accessible to all populations, and the disparities in quality of care need to decrease. Health care providers need to evaluate access to care, treatment quality and its effectiveness. Meanwhile public policy needs to support funded programs that will improve access to care and support preventative services.
According to the federal Agency for Healthcare Research and Quality, they have assessed the nation’s health system annually since 2003, reported that, in 2015 the health care delivery system has made progress to achieve the three aims of better care, smarter spending, and healthier people (City of White Plains Health Equity Report, 2017). Health disparity is one of the major concerns in the provision of quality care and access to healthcare which directly the life expectancy of the nation as about ethnicity and race. However, describing the health outcomes or status of an ethnic group in the population would help in a better evaluation of the disparities that occur within minority groups in our society. “Racial/ethnic disparities in health and quality of and access to health care are a well-documented and persistent problem. Across many indicators of health, access to care, and health care quality, racial/ethnic minorities fare worse than whites, and each population faces specific challenges”(James et al., 2017, p. 1).

In the US “barriers generally stem from forces within the organizational environment of the health care delivery system or within the broader social system itself” (Barr, 2011, p. 273). In recent discussions of health care disparities, a controversial issue has been whether racism is the cause of health care disparities or not. On one hand, some argue that racism is a serious problem in the health care system. From this perspective, the Institute of Medicine (IOM) states that there is a big gap between the health care quality received by minorities, and the quality of health care received by non-minorities, and the reason is due to racism. On the other hand, however, others argue that health care disparities are not due to racism. In the words of Sally Satel, one of this view’s main proponents, “White and black patients, on average don’t even visit the same population of physicians” (Satel 1), hence this reduces the chances of racism being the cause of health care disparities. According to this view, racism is not a serious problem in the health care system. In sum, then, the issue is whether racism is a major cause of health care disparities as the Institute of Medicine argues or racism is not really an issue in the health care system as suggested by Sally Satel.
Despite the substantial developments in diagnostic and treatment processes, there is convincing evidence that ethnic and racial minorities normally access and receive low quality services compared to the majority communities (Lum, 2011). As such, minority groups have higher mortality and morbidity rates arising from both preventable and treatable diseases judged against the majority groups. People of color face inequality and intersectionality in healthcare whether through insurance coverage, access, social economics, and quality of care. This leads to our health care system having disparities. It remains a big challenge today as 41% of people living in the United States are people of color (kff.org). People of color aren't getting the same care as someone who's white. Racial and ethnic minorities received lower care. Race also plays a role if someone will be uninsured or not.
