The National Healthcare Quality and Disparities Report (NHQDR) (2012) identified three key themes. The themes are health care quality and access are suboptimal, especially for minority and low income groups; overall quality is improving, access is getting worse and disparities are not changing; and urgent attention is warranted to ensure continued improvements in: quality diabetic care, maternal and child health, adverse events, disparities in cancer care and quality care among the states in the south.
The NHQDR 2012 is a comprehensive report that implies there are changes that need to occur at multiple levels within the health care system and public policy. The report implies that the health care system needs to become more accessible to all populations, and the disparities in quality of care need to decrease. Health care providers need to evaluate access to care, treatment quality and its effectiveness. Meanwhile public policy needs to support funded programs that will improve access to care and support preventative services.
As a health care provider Advanced Practice Registered Nurses (APRNs) should be actively involved in working towards eliminating disparities. APRNs should conduct research to determine the health disparities in their community by utilizing the state and local database for the Department of Health. The APRNs need to report the findings to local health care providers, and political leaders, then initiate change based on the findings.
APRNs could write culturally relevant health promotional literature on diabetic disease management and make the literature available at public health clinics, primary care clinics, local parishes and senior centers. The literature needs to be available in multiple languages ...

Planning and implementing a breast screening event in a predominantly African American community is one way the ARPNs are able to address disparities in preventative care. The event needs to be publicized at local social centers, parishes and other religious establishments. APRNs should collaborate with local health care providers in order to ensure follow up care is available to address any clinical findings.

The United States’ population is currently rising exponentially and with growth comes demographic shifts. Some of the demographics shifts include the population growth of Hispanics, increase in senior citizens especially minority elderly, increase in number of residents who do not speak English, increase in foreign-born residents, population trends of people from different sexual orientation, and trends of people with disabilities (Perez & Luquis, 2009). As a public health practitioner, the only way to effectively eliminate health disparities among Americans, one must explore and embrace the demographic shifts of the United States population because differences exist among ethnic groups (Perez, 2009). We must be cognizant of the adverse health conditions for each population and the types of socioeconomic factors that affect them. Culture helps shape an individual’s health related beliefs, values, and behaviors. It is more than ethnicity and race; culture involves economic, political, religious, psychological, and biological aspects (Kleinman & Benson, 2006). All of these conditions take on an emotional tone and moral meaning for participants (Kleinman & Benson, 2006). As a health professional, it is one’s duty to have adequate knowledge and awareness of various cultures to effectively promote health behavior change. As of today, there are many programs and efforts being made that have either already decreased the gap or are attempting to bring change to the problem of increased deaths of African American women from breast cancer. One example is a study that was done in Massachusetts that gave low-income African American women aged 50-70 resources and education for six years, and it was “concluded that the Massachusetts program appeared to mitigate the disadvantages of living in high-poverty neighborhoods” for the incidence of breast cancer in that specific area (Cunningham 595). This study shows that these women need help that has not been previously provided to them in order to reduce the disparity. In this regard, the role of affordable health care needs to be available in order to decrease this problem. The same study showed that “among women without health insurance, disproportionately large numbers are [older African Americans], providing an explanation for high rates of advanced stage cancers at presentation among [African American] women in general” (Cunningham 594). If women are to be able to access affordable screenings, affordable health care must also be provided. Once again, this brings in the role of government in the lives of African American women. Federally qualified health centers offer preventative health care and screenings for a reduced or free cost to women of low socio-economic status, many of which happened to be African American women at a particular clinic, and it was found that the incidence of breast cancer in that community was reduced from the rates that were established previously (Adams 640). Healthcare disparities are when there are inequalities or differences of the conditions of health and the quality of care that is received among specific groups of people such as African Americans, Caucasians, Asians, or Hispanics. Not only does it occur between racial and ethnic groups, health disparities can happen between males and females as well. Minorities have the worst healthcare outcomes, higher death rates, and are more prone to terminal diseases. For African American men and women, some of the most common health disparities are diabetes, cancer, hypertension, cardiovascular disease, and HIV infections. Some factors that can contribute to disparities are healthcare access, transportation, specialist referrals, and non-effective communication with patients. There is also much racism that still occurs today, which can be another reason African Americans may be mistreated with their healthcare. “Although both black and white patients tended not to endorse the existence of racism in the medical system, African Americans patients were more likely to perceive racism” (Laveist, Nickerson, Bowie, 2000). Over the years, the health care system has made improvements but some Americans, such as African Americans, are still being treating unequally when wanting the same care they desire as everyone else.
According to the federal Agency for Healthcare Research and Quality, they have assessed the nation’s health system annually since 2003, reported that, in 2015 the health care delivery system has made progress to achieve the three aims of better care, smarter spending, and healthier people (City of White Plains Health Equity Report, 2017). However, they continue to promote health equality and reach the goal of New York State being the healthiest. Advanced practice registered nurses play a significant part in extending access to health care by providing primary care and specialty care services to clients. Advanced practices registered are mentors, educators, researchers, and administrators. According to Health Resources and Services Administration, “Ninety-six percent of the NP workforce reported being in clinical practice, providing direct patient care” (Health Resources and Services Administration 2016). Furthermore, “Nearly three percentages were in faculty positions and approximately one percent was in administrative positions”(Health Resources and Services Administration 2016).
Cultural Competence is important for many reasons. First, it can help develop culturally sensitive practices which can in turn help reduce barriers that affect treatment in health care settings. Second, it can help build understanding, which is critical in competence, in order wards knowing whom the person recognizes as a health care professional and whom they views as traditional healer, can aid the development of trust and improve the individual’s investment and participation in treatment. In recent discussions of health care disparities, a controversial issue has been whether racism is the cause of health care disparities or not. On one hand, some argue that racism is a serious problem in the health care system. From this perspective, the Institute of Medicine (IOM) states that there is a big gap between the health care quality received by minorities, and the quality of health care received by non-minorities, and the reason is due to racism. On the other hand, however, others argue that health care disparities are not due to racism. In the words of Sally Satel, one of this view’s main proponents, “White and black patients, on average don’t even visit the same population of physicians” (Satel 1), hence this reduces the chances of racism being the cause of health care disparities. According to this view, racism is not a serious problem in the health care system. In sum, then, the issue is whether racism is a major cause of health care disparities as the Institute of Medicine argues or racism is not really an issue in the health care system as suggested by Sally Satel.
Cultural competence in health care provision refers to the capacity of health care systems to offer good care to patients and accommodate employees, who have diverse beliefs, behaviors, and values to meet their cultural, linguistic, and social needs. It comprises of policies, attitudes, and behaviors that integrate to form a system that can operate efficiently in cross cultural conditions. Healthcare organizations look at cultural competence from two major viewpoints. Firstly, it is a tool to enhance patient care from all backgrounds, social groups, languages, religions, and beliefs. Public Health Nursing (PHN) aims to improve the wellbeing of the population by promoting health and preventing disease among all people in the communities (Public Health Nursing, 2013). The PHN utilizes the primary, secondary, and tertiary prevention to help improve the health of the communities. The PHN process is applied to all levels of practice. Interventions are “actions take on behalf of individuals, families, systems, and communities to improve or protect health status” (Stanhope & Lancaster, 2012, p. 191). This paper will identify PHN interventions such as screening, outreach, and referral and follow-up, health teaching, and counseling that was identified in the PHN in the 21st Century project that this author completed as part of the PHN experience. This paper will also identify if the interventions were at the community, system or individual/family level.
