Often time, nurses has been viewed by patients, their family members and the medical team as basic emotional care givers, pill crushers or cart pullers and not as healthcare professionals who are more interesting in health promotion, disease prevention and better patient outcomes. They also often forget the emotional, physical, mental, and caring part that is involved with the profession. And to make matters worse, nurses are continued to be viewed as a threat by doctors more than ever before especially with the opening of Nurse Practitioners programs.
The hospital that I was working at did not have a specific policy about compassion fatigue. However, there are a number of professional documents and organizational policies that inform the discussion of compassion fatigue including the CNA code of ethics and the employment standards act. In the next few paragraphs I will discuss how each one informs the discourse of compassion fatigue.
Freud’s approach trauma is based in the treatment of hysteria. According to Ringel and Brandell, Freud and Breuer, considered an “external event” as responsible of determining hysterical symptoms. The common component between hysteria and trauma is the outcome of fright. Freud and Breuer emphasis the importance of cathartic experience as a way of decreasing or vanishing the effect. The “cathartic method” that was developed by Breuer, assisted to release of inhibited emotions. Freud believed that the libido, necessary to be relished for the symptoms to be improved (p. 43).
It reflects trust, intimacy, and responsibility, which are elements essential to any nursing relationship. It is the core of nursing. In the article, “Dimensions of Caring: A Qualitative Analysis of Nurses’ Stories”, an analysis was done on over two hundred stories submitted by nurses around the world, illustrating the point that the practice of nursing encompasses much more than just technical skills. In one of the stories, a nurse stood by the side of a young mother grieving the loss of her baby with nobody else to turn to. This nurse accompanied the 19 year old mother to the cemetery the day after the baby’s death, New Year’s Eve, to bury her baby that had been born at just 22 weeks gestation. She bought her a book on grieving and loss and supported the woman during that difficult time. This nurse did not have to do that—she did not have to go above and beyond for this stranger. But she did. She felt empathy and compassion for this woman and she acted on that. That is what nursing is about, going above and beyond, making emotional investments into the lives of other people. The significance of the actions of this nurse cannot be overstated. The definition of compassion is “sympathetic consciousness of another’s distress with a desire to alleviate it” (Merriam-Webster dictionary, 2011). Compassion and the desire to nourish may be the deciding factors that would lead one to pursue a career in nursing. Nurses over the span of their career will have extensive exposure to trauma, pain, and unfortunate situations. Workplace stressors such as scheduling and increasing workload along with repeated exposure to the hardships of others predisposes caregivers, especially nurses, to develop a unique type of burnout labeled compassion fatigue (Joinson 1992). Compassion fatigue develops when a nurse unintentionally takes on the misfortune, anxiety, pain, and trauma of the patients they care for.

A theory that should be implemented to more effectively promote professional growth and development is Orem’s theory of self-care deficit. The pediatric oncology unit has become a heavily studied area for those interested in prevention of compassion fatigue, burnout, and identification of those character traits that offer increased resilience. Nurses are expected to maintain professionalism and appropriate work-life balance but this may become a difficult task on a unit where children have a chronic, intensive, and potentially life-ending condition. Nurses become close to the patient and their family and when death occurs they too may feel a great sense of intense grief and loss. The acknowledgment of this grief and the promotion of adequate self-care habits, work-life divisions, and the ability to recognize when help may be needed are amongst the most important means in prevention of burnout and compassion fatigue. In addition to utilization of positive coping skills by the nurse a responsibility by the organization is also necessary to prevent staff burnout and turnover. The organization is responsible for acknowledgement of a loss on the unit. My personal philosophy of nursing is related my life experience and my personal philosophy of life. Using reflection-on-action, I have begun to understand the influences that have lead me to nursing. I discovered client and family centered care to be an important quality when I look at the influential nurses in my life. To develop a positive therapeutic nurse-client relationship, nurses must integrate all 5 dimensions of the therapeutic nurse-client relationship into their practice (CNO, 2006). When it comes to providing client-centered care, the dimensions of trust, empathy and respect are particularly important (CNO, 2006). Illness can be a traumatic experience for patients and their families, and it is important to be empathetic to the patient’s needs, while still trusting the patient to be an expert in their illness and care.

Since rehab nurses frequently have the same patients for long periods of time, they witness the patients progress made from when they first began their treatment. Although they may feel happiness once their patient reaches their maximum function, they are also susceptible to compassion fatigue during treatment. Rehabilitation Nurses are constantly exposed to patients who have lost a part of who they are due to physical impairment and who need guidance in finding coping mechanisms. As a result, they are expected to replenish patient enthusiasm and provide constant support.
