A community assessment is “an effort to identify and prioritize a community’s health needs, accompanied by collecting and analyzing data, including input from the community” (Clark, 2015, p.357). Community assessments are important in planning to improve and promote health in the community. This community assessment focuses on the community of Lake Andes, South Dakota. A windshield survey is “an informal means of collecting population assessment data by driving through an area and observing its visible features” (Clark, 2015, p.359). To address the physical environment, some of the things that look well in the community would be that most of the businesses look well kept, and in the summer time, there are lots of trees and grassy areas. One would not say that air quality looks like it would be an issue. The town does not have any factories or anything that looks like it would really pollute the air. One thing that is concerning is the amount of garbage and trash that a person finds laying around town. If a person drives down the road and looks in the ditches along the highway that runs through town, they are just filled with trash that people throw out of their car
The Englewood community is one of Chicago’s 77 official communities. The community is an urban setting comprised of 30,654 residents. Ninety-eight percent (98.8%) are African American. Of the population, 5,740 are youth 10 to 19 years of age, 18.7% of the population and approximately 60% of the households are headed by single women (U.S. Census, 2010). In 2011, median household income was $24,049 compared to the City of Chicago median household of $43,628. The 2010 U.S. Census did not report household income. Englewood ranks as one of the highest poverty communities in Chicago with a poverty rate exceeding 39% (Atlas of Illinois Poverty, 2003).

A Community Health Needs Assessment (CHNA) was conducted in the city of Atlanta, Georgia, with the focus on mainly Dekalb and Fulton Counties. The CHNA was conducted to identify the needs and resources in the community with the input from the community members, key stakeholders, and the public health. The CHNA would be used as a guide for the community in its future community health projects.
For this study ten participants were chosen to complete the study. For this particular study, the participants had to be the eldest and youngest child from the same family. They both also had to be raised in the same household. The pairs were picked at random and then asked to complete the test. There were three males tested and seven females tested.
According to Allender, Rector, and Warner (2014), public health is a combination of both an art and a science (2014). The mission of public health nursing is to promote health, prevent disease and ultimately prolong life (Allender et al., 2014). In order for this to occur an assessment must take place. An aggregate or community assessment begins with a collection of data. This includes: the community’s health needs, risks, environmental conditions, financial resources through local census data, and a windshield survey (Allender et al., 2014). Through public health nursing, communities can collectively come together to help promote an overall better health standing.
For our group community assessment, we assessed the neighbourhood of Moss Park. Throughout our assessment at Moss Park, we noted many of the community’s characteristics including physical environment, resources available, strengths and weaknesses etc… Moss Park is a neighbourhood located in downtown Toronto, the area is mainly comprised of worn-out buildings and houses with a satisfactory number of resources including a public library, school, health care centers, pharmacies etc… During the assessment of this neighbourhood we noticed that the physical environment was contaminated with the presence of garbage on the streets and in residential areas.
