According to what I read in the textbook, the CMS 1500 form is divided in two considerable parts: 1- Patient and Insured Information, which you can find in locators 1 through 13; and 2- Physician or Supplier Information, which you can find in locators 14 through 33. But this form can be divided in three sections as well: 1-Demography (from locators 1 through 13), 2- Procedural and diagnostic information (from locators 14 through 24) and 3- The technical part (from locators 25 through 33).
In locators 1 through 13 you will find: Type of Insurance (you will write down which type of insurance the patient is using. It could be a government type of insurance, group health or other) , Insured’s I.D. Number, Patient’s Name, Patient’s Date of Birth/Sex,

Therefore, CMC is experiencing an ALOS of 9.2 days with the statewide average being 6.4 days. Expenses per patient day are highest at CMC with an exception of 2 larger university medical centers. There is currently a Governing Board of 27 members and a Parenting Corporation of 19 members. Both boards lack diverse environment.
Phase I addressed basic statutory definitions, general prohibitions, and explanations of what constitutes a financial relationship between a physician and a health care entities providing DHS’. Phase II deals with the regulatory exceptions, reporting requirements, and public comments pertaining to Phase I. Finally, Phase III Final Regulations were published in September of 2007, and largely addressed comments made after publication of the Phase II rules and regulations. It also reduced some of the regulations placed upon the healthcare industry by explaining and modifying some of the exceptions related to financial relationships between physicians and DHS entities where there is minimal risk of abuse to the patient, Medicare or Medicaid.

HIPPA encourages the use of electronic medical records and the sharing of medical records between healthcare providers, because it can aid in saving lives. HIPAA requires that patients have some knowledge of the use of their medical records and must be notified in writing of their providers' privacy policies. HIPAA has technical requirements that a healthcare provider, insurer, or service provider, unless exempt under state law, must provide. An organization must conduct a self-evaluation to learn what threats its records face, and develop techniques needed to protect the information (HIPAA, 1996).

...0. CMS-1500 is the basic form that has been set by Center for Medicare and Medicaid services and is used by most outpatient clinics. CMS-1450 is the form that is used hospitals to claim reimbursement for hospital visits. While CMS-1500 is used for patients who are under Medicare Part B, CMS-1450 is used for patients insured under Medicare Part A. Some of the charges that need to be claimed using CMS 1500 are ambulatory surgery performed in a certified Ambulatory Surgery Center, all hospital based clinics, and hospital based primary care office. Furthermore, some of the charges that need to be claimed in CMS-1450 are emergency department visits, ancillary department visits, outpatients services such as infusion therapy or observation, all services rendered during an inpatient visit, and any pathology service provided regardless of patients’ presence (Ferenc, 2013).
Martin Cash the infamous bushranger lived in the 19th century. He was born on the 11th of October 1808 at Enniscorthy Country Wexford, Ireland and was the son of George and Mary Cash. He was a farm boy but his family were fairly well off in terms of money. Growing up, he did receive a reasonable standard of education but was an irregular attendance at school. This was partly due to his lack of interest and the fact that he was expelled three times by different masters. Before being sent to Australia, he had many occupations such as auto biographer/memoirist, cattle herder, market gardener and as a low-life thief. Then something happened that was to change his life forever.

He was then replaced by the leader of the Conservative party Stephen Harper. Paul Martin was a great Prime Minister compared to Stephen Harper. Stephen Harper was boring, mean and selfish. He lost the Country billions. Martin on the other hand brought Canada to places we've never been.
It serves as a source for planning patient care and the services provided to that patient. Medical records begin from when the patient was born. It contains diseases, illnesses and whatever the patient tells their physician about his or her past and present status. It also contains lab test results, medication that was ever prescribed. It also contains allergies, referrals ordered to other health care providers and plans for further care.

The three mathematicians I chose are as follows: Johann Heinrich Lambert, Evariste Galois, and David Hilbert. Johann Heinrich Lambert was an 18th century mathematician, and his contribution to trigonometry was providing evidence that “Pi” is irrational. His contribution was important because “Pi” is used for finding the circumference of a circle to its diameter. In addition, Evariste Galois was a 19th century mathematician, and his contribution to trigonometry was discovering the theory of polynomial equations. His contribution was important because he proved that there is no general algebraic method for solving polynomial equations of any exponent greater than four. Lastly, David Hilbert was a 20th century mathematician, and his contribution included more complex trigonometry problems. He discovered a new formal set of geometrical axioms, known as Hilbert’s axioms. Also, he showed that there were an infinite number of possible equations.
This week, I was able to learn more about the LGBT community through understanding LGBT media and theater. Over the past century, many Gay playwrights like Terrence McNally, inspire their audience members through portraying their LGBT identity on televised shows and movies. Over the past years, the LGBT community has been oppressed and ostracized by other people. The LGBT community was often criticized for so many social issues in the past century. Many people in the LGBT community fought for certain rights, such as same-sex marriage, civil unions, medical hormone replacements, and many other rights. Over time the LGBT community has expanded their ideas through theater arts. Thus, in this assignment, I will provide a brief evaluation the history and personal background of LGBT playwright named Terrence McNally. By evaluating McNally’s background, we will be able to familiarize ourselves with the perspective of an LGBT individual in television.
