For my observation, in the health care setting, I shadowed Nancy McAuley. She is a licensed speech-language pathologist. She works for the Mon Valley Hospital, but provides therapy at the outpatient rehabilitation facility that is still though the Mon Valley Hospital. This is for patients that were recently in the hospital, but is home bound now but still need some type of therapy. At the facility there is physical, speech, and occupational therapy. I shadowed her when she was at the Mon Valley rehabilitation center. I was able to watch two different types of clinical focuses. The first patient that I was able to observe had many clinical focuses that include apraxia, aphasia, and dysphasia. The other patient that I observed was working on cognitive tasks to work the brain on the right and left hemispheres. The speech-language pathologist said she does not have a particular therapy approach that she used but focuses on the functional aspects of task that the patient is doing. She focuses more on the functional everyday functions that individuals do rather than working on a task that that they will just become frustrated. During the therapy session, they administered VitalStim therapy. VitalStim was used for the patient because he was suffering with difficulty swallowing also known as dysphasia. Electrical stimulation is used to aid muscle strengthening and muscle recruitment to rehabilitate the swallow. The clinician applied the VitalStim while working with the patient on swallowing exercise. To make this happened she has the patient read one to two pages out a book to get the VitalStim to be effective. Today’s clinical experience truly affected me in multiple ways. I went into this day with an open mind, and was pleased with the patients and the way I was able to conduct myself. This clinical affected me because throughout the day I felt that I experienced many emotions. A few times during my day I did have to fight back tears. I felt I had this emotion because some of the individuals expressed how they wanted to get better in order to get home to their families.
Having worked as a rehab aide in an outpatient clinic, my pediatric observation experience was completely different from what I am used to seeing. The therapist I observed was Allie Ribner who works at All Children’s Child Development and Rehab Center. Each session was completely different from one another for the session was geared towards the goals of the child and families. I found this to be a great learning experience for I saw a wide variety of different treatments and age range from 14 months to 15 years old.
Regina Geis is a ten-year old female who is diagnosed with celiac disease. No one in her immediate family has been diagnosed, including her parents and her younger sister. Celiac disease is an autoimmune disease of the digestive system that is usually indicated by long-lasting inflammation in the small intestine that occurs when a protein from the plant called gluten, which is usually found in grains such as wheat, barley, and rye, is consumed, resulting in an interference of absorption of nutrients from food (Calvo, Soriano del Castillo, & Vinuesa, 2012.) Regina was diagnosed with celiac disease when she was five. After she was diagnosed, she was put on a gluten-free diet.
The patient is a 78-year-old gentleman who is brought to the emergency room because of increasing confusion. Evidently the daughter has taken to the bank to get some money when the daughter try to assist him to put his money in his pocket he became aggressive and combative and began to swing at her with his cane and then walked off. She was the unable to find for approximately 4 hours. When he was found he was brought to the emergency room. In the emergency room the patient was placed initially in observation status. Despite being treated in observation with fluids he remained confused and somewhat aggressive and it was determined that the patient required acute inpatient hospitalization. The foundation to being a good coder is having a thorough knowledge of medical terminology. When we communicate with other people in the healthcare field, the universal language used is medical terminology. When we abstract information from the doctor's notes to code, they will be using medical terms. If we do not have an understanding of the terminology, we could code for the wrong thing and have the claim denied.

However, I believe the one that stands out the most is communication. To prosper in the medical field you have to be able to understand doctors and communicate with co workers. I had the opportunity to observe the surgical ward in Seremban Hospital. What really overwhelmed me was how the ward actually looked disorganized when patients' families visited during lunch time. One of the patients, who was undergoing vacuum dressing, was resting on the bed next to a partition. He was hardly noticeable from far because there were two trolleys blocking the view around his bed. One was piled with patients' charts and another one had been forgotten to be removed by the nurse when she rushed to attend another patient. It was a reckless thing to do but it made me see that the nurse must have had so much on her plate to be negligent about this seemingly-insignificant thing. The third observation site I visited was my professor, Dr. Saltarrelli’s private clinic Autumn Oaks. Her clinic is a relatively new clinic, but the area around the building is old. Her clients range from toddlers to adults. Some of her clients see her for either speech therapy or for audiology. The clinic is quite small; there was a small hallway with a trophy case displaying equipment used for hearing aids and some pictures. Along the hallway there was a designated therapy room, two offices, a restroom and a kitchen. The therapy room was quite small it contained one adult sized table with two chairs, a children’s table with two chairs, a set of hanging cabinets, and a huge storage cabinet filled with board games and other play equipment.
I enjoyed this assignment of observing a health care facility. I decided to observe the atmosphere of Mercy Hospital Emergency room. When observing the emergency room, I decided to take a seat to get a better look at the environment. The emergency room was very quiet. When you first walk into the emergency room there is a security desk for visitors and a check in center for the patients. The dorms are always locked, and you need to check in with security to get in.
