My Capstone Proposal Project will be about sleep disturbances associated with cardiac surgery patients. My proposed strategy consists of the idea that by increasing nursing education and intervention there will be a reduction in sleep disturbances for the cardiac surgery population. There are many reasons why surgeons, quality improvement groups, and even nursing associations, such as Preventive Cardiovascular Nurses Association, would be interested in my Capstone Proposal Project. There are currently three cardiothoracic surgery stepdown units within Duke University Hospital. Depending on the nursing managers’ thoughts, the project could spread to other cardiac units and intensive care units that care for cardiac surgical patients. Duke University Hospital has one cardiothoracic surgery intensive care unit, but there are other intensive care units that take care of this population as well. I would seek help from the nursing management teams and nurse educators to permanently sustain educational training requirements for the nursing staff providing direct patient care to the cardiac surgical population. The Patient Satisfaction Committee would communicate tips and training into monthly meetings with staff and managers will update staff monthly regarding the Hospital Consumer Assessment of Health Plans Survey (HCAHPS). As student nurses, I have many goals that would like to achieve upon completion of 150 hours of my capstone course. My plan is to use this opportunity to demonstrate competency skills, and knowledge gained through previous quarters. I have listed seven learning goals that I wanted to achieve during the capstone experience. On my first day of capstone, I have gone over each one with my preceptor and requested from her to provide me with feedbacks, so I can use them as stepping tool to work upon. These goals ranged from critical thinking decision making, culture, growth & development, legal and ethical to clinical competence safety/holistic care, evidence-based practice, and professionalism lifelong learning.
The ability of a unit to survive is largely dependent upon the hospitals internal financial budgetary performance and the external needs within the community. Developing a financial budget is a process that should use teamwork to plan and implement in order to be effective. The budget sets perimeters for administrators to follow throughout the year, allowing the director to report variances while providing guidance to maintain a minimum variance and adjust when possible (Finkler & McHugh, 2008). By using all department managers in the planning process of the new budget, the nurse executive is able to develop effective strategies for all departments while investing in the goals. This eliminates many problems associated with budget and identifies areas that need improvement or expansion. Because of the competition, declining margins, and other economic pressures, nurse executives need to take steps to control costs and increase revenues for this unit. The overall goal of the financial performance within the organization is to meet the total budgetary needs of the unit to produce favorable outcomes. My focus will be to propose the expansion of a new Joint Replacement Unit (JRU) within the hospital, while identifying the major operating components of the budget for this organization. The importance of reviewing the budget for a newly developed unit is to allow the nurse executive and administrative team to manage the existing organizational programs within in the facility, plan for goal accomplishments for the new unit, while controlling costs.
Nursing is ever-evolving. Healthcare in general is becoming more sophisticated, which requires an increase in knowledge and education, as well as the need for nursing staff to be able to think critically. As medical advances are at an all-time high, reimbursement from state and private insurance has decreased. These factors lead to a greater need of autonomy and evidence-based research by RN’s (Huston, 2014)

They can engage in research to Generate evidence-based practice and help shape the quality and safety through collecting and tracking data to improve care. Nursing research helps create the scientific foundation for clinical practice, prevention, and improved patient outcomes. Nurses can Redesign nursing education (Nursing 's role in health care reform - American Nurse Today. n.d.). All nurses need to maintain basic competencies to meet the requirements of an aging and diverse community, with an emphasis on clinical training in multiple settings across the lifespan. The emphasis should be on quality and safety, evidence-based practice, research, and leadership (Nursing 's role in health care reform - American Nurse Today.

As long as nursing continues to grow, there is no limit to the achievements that can be accomplished in the profession, for not only the individual patients, but also for the healthcare industry as a whole. Nurses must remember, that as they move forward, they will continue to break down barriers and stake their claim as a necessity in healthcare. Nurses must also hold tight to the teachings of their predecessors. The future of nursing holds great advances in healthcare and technology innovation; but, most importantly, the future of nursing needs to be saturated with nurses that are willing to put their heart and soul into healing the human spirit. The future of nursing must bring with it the teachings of the past, while providing their patients with the most qualified nursing care.

The goal of my learning plan for this semester is to develop skills in health education specific to adolescents. I chose this as my learning goal because my placement this semester at Sunnybrook Hospital, Injury Prevention Program, gives me an opportunity to provide health education to high school students every Tuesday at out P.A.R.T.Y. Program so I thought I should use this opportunity to develop my and improve my nursing skills in this area. As well as good health education skills to adolescents can benefit the students by proving them a motivating learning environment and better learning outcomes. I hope they can enjoy my teaching and absorb the information and use them in the future. I have considered many factors in the process of developing my learning plan such as the nursing standards of health promotion, prevention and health protection, professional relationships, capacity building etc… I brainstormed possible topics such as possible topics that are related to injury prevention itself for example, concussion prevention or preventing sports related injuries. I also thought about doing topics that are relevant specifically to the P.A.R.T.Y. Program such as teenage suicide and prevention or distracted driving. After assessing my placement and the clients which in this case are the students, I have recognized that one of the program’s goals is to educate the students about injuries and injury prevention. I also found that our clients had a lot of potential in learning about this subject which can greatly benefit their lives and influence change in their behaviour to increase control of their own health and make better choices. Several years ago in 2008 the RWJF and the IOM collaborated for two years to discern the future needs of the nursing profession. Most importantly, the objective was to outline the critical actions needed to ensure nursing was ready to seamlessly move towards the future. This was no easy task as nurses work in such diverse settings such as outpatient areas, acute care settings, the community, and long term settings to name a few. Couple this with the fact that nurses have a variety of educational avenues such as the associates, diploma, or bachelor’s degree open to them to achieve the status of registered nurse (Institute of Medicine, 2010). All of this considered, the committee did design four key messages regarding the future of nursing as key in the transformation of health care as evidenced in their "Future of Nursing" report.
Issues and trends in the nursing profession are constantly changing due to discoveries in technology and research, changes in reimbursement and legislation, and the changing needs of our population; in fact the only constant in healthcare is change. This course has provided me with a foundation and better understanding of the need for higher education to empower nurses to become leaders and help shape the future of the nursing profession. Nurses are the “backbone” of healthcare and this is becoming more evident with the changes we are currently seeing in the healthcare industry.
