Beds should provide comfort safety, and a sense of protection. Bedrails are a feature that is meant to provide safety from falling out of bed for patients/residents who are not completely mobile. Bedrails have become a question of controversy because they can be considered a restraint and even pose a risk of injury or death when patients/residents become entrapped in the rails. Reports of incidences where a patient was entrapped in the bedrails or fell to the floor when the bedrails collapsed was included in the study. According to Hignett and Griffiths (2005) split-side rail entrapments only accounted for 5% of the reported incidents. Out of a total of 794 reports, 396 entrapments resulted in deaths, 292 resulted in injury, and 53 in near misses. 355 of those reports included the type of bedrail, 199 split rails, 105 half rails, 37 full rails, and 14 three-quarter rails. Patients were often found with their head either between the rails or in the gap between the rail and either the head or foot board (38%). Following a collapse of a rail 22% were found on the floor. 18% of patients were found between the rail and the mattress and 14% were found with their upper or lower limbs in the rail. In relation to the lecture content covered in this week’s lectures, I felt they related to clinical when describing the layout and description of the milieu. For example, I was able to see how the doors were securely locked, and the nursing station was behind an encasement called a bubble. Seclusion and restraint started out in psychiatric hospitals and have now evolved into many schools. Restraint started out in England in the mid 19th century after having a history of poor conditions. Since Americans did not open up their first state- run mental hospital until 1822, they were unaware of the negative history that happened during the British reformation ("Human Side of Hospitals"). The American physicians thought that the restraints were keeping their patients safe when it was actually mistreatment of their patients. Anything that can be used to restrict the movements of a patient is a form of restraint. Things used as restraints can be leather or velcro wristlets or anklets that are used to hold the patient or attach them to their bed, lock them in their room, or by using sedating chemicals.
The definition of grit is simple; it means being passionate about long term goals, and showing stamina to pursue them, but should students in public schools have a class dedicated to a grit-focused curriculum? While administering pain medication, it is important for this author to assess pain based on the pain scale of zero to ten, location of pain, and reassessing pain in 30 minutes after the medication was administered. Another situation that is important for the nurse to learn is to follow up on lab draws for medication that requires peak and trough levels in order to either administer or hold the next dose of the medication. Also, some of the residents thought that this author has the rehabilitation floor in control. In an article written by Samantha P. Ziglar, BSN, RN, she describes the main purpose of restraints through her eyes in the medical field. Ziglar states that at least one in every four patients will be restrained at least once, that’s 79.715 million people in the United States (Population). “Specific reasons include fall prevention, limiting wandering behavior and preventing dislodging or interference of therapeutic devices, and controlling violent or agitated behavior” (Ziglar 1). Ziglar emphasises the growing problem among restraints; improper use among nurses. “Patient safety should be every nurse’s top priority; therefore, nurses need to have a thorough knowledge base of the risks and benefits of using physical restraints” (1). As a result of what a nurse needs, restraints are required to succeed in his or her profession, making their patient as safe and comfortable as possible. Have you ever had to make a decision in your life? I’m pretty sure we all have, the decision can even be what you ate for breakfast in the morning. Robert Frost wrote a poem about a man who has to make an important decision. In the poem “The Road Not Taken”, by Robert Frost, he states, “Two roads diverged in a yellow wood and I - I took the one less traveled by, And that has made all the difference” (Woods). To me taking the road less traveled by means, doing something on your own. It means not following a crowd, it means doing something unique. It can be hard to go your own way, but that is what Robert Frost is talking about. Everybody should go the path is right for them, not the one that many other people have gone down. Safety is a primary concern in the health care environment, but there are still many preventable errors that occur. In fact, a study from ProPublica in 2013 found that between 210,000 and 440,000 patients each year suffer preventable harm in the hospital (Allen, 2013). Safety in the healthcare environment is not only keeping the patient safe, but also the employee. If a nurse does not follow procedure, they could bring harm to themselves, the patient, or both.
