1. How will you encourage Janis’s participation in reaching the goal?

I would encourage Janis by first letting her know that she is not alone in the process. I would also encourage her to set her own goals. Highlighting her strengths would also be used to encourage Janis to participate in reaching her goal. I would be sure to include Janis throughout the process and providing encouragement along the way. I would encourage Janis to complete the tasks that she is capable of completing and not rely heavily on others. I would always ask Janis what she thinks or how she feels regarding any decisions. Janis would know that she is ultimately in charge of reaching her goal.

2. How will you make the referrals? Do you anticipate any barriers? Explain.

Once it is determined that I am unable to provide the services that Janis need, I would begin the referral process.

First I would determine Janis needs and the agencies that can provide the needed services and her eligibility. I would make initial contact with the facility and give them a brief history of Janis case as well as make sure services are available prior to giving Janis and her mother the contact information. I would ensure the facility had someone available to assist Janis and her mother throughout the application process if needed. I would also ask Janis or her mother to call me with the appointment information. I would follow-up after the appointment.

Possible barriers would be past unfavorable history with certain agency. Other agencies not provide feedback and documentation of the services rendered to Janis.

3. How will you monitor service provision? What is YOUR plan? (List your goal and objectives for monitoring service provision.)

4. Describe the criteria you will use for the evaluation of service provision.

The text states, “Monitoring of services goes most smoothly if close contact is maintained” (Woodside & McClam, 2013, p. 310). I will continue to make documentation regarding Janis progress both inside and outside agency. By evaluating the service provided, I will be able to decide if the correct referral was made, make necessary changes if needed and determine if Janis reach her goal.

5. Describe a situation where you will advocate for Janis.

Janis being a minor may not understand what is actually needed or understand the process and choices she would have to make. I would speak on her behalf during the initial contact with medical examiner, counselor and during the referral process by contacting the outside agency. I would inform her them her history of defiant behavior, violence, and possible drug usage as well as her desired outcome.

6. Will you be a broker or a mobilizer? Explain.

I would be a broker. It is obvious that Janis will need multiple services. As a broker I would be able to act as a liaison between the different agencies.
