Throughout the Think Culturally assignment, I realized the impact I will have as a healthcare provider. The scenarios that were provided throughout each course assignment gave me insight on the negative health outcomes that can occur when cultural competence is not present. In order to have cultural competence, I must acknowledge that some patient’s health beliefs will be different from my own. This can be distinguished by having open communication and asking pertinent questions. Treating patients that are not proficient in English, can become overwhelming. As a future nurse practitioner, I want to utilize all the necessary recourses to ensure my patient’s fully understand the plan of care and treatment regimen. The Think Culturally assignment
offered several tips and resources healthcare providers can utilize while interacting with minority patients. I plan on using the LEARN mnemonic within my practice. This stands for listen, explain, acknowledge, recommend and negotiate. As a provider, using this mnemonic will ensure that proper communication occurs throughout the entire visit. I also plan on using a face to face interpreter. I think this is a key resource that must be used while interacting with minority patients that are not proficient in English. Not only is cultural competence important for providers, but also for all staff members that are interacting with patients. It is imperative they too receive the same education and training. Within my clinic, I would have yearly competency workshops that would be mandatory for all staff members to attend. These workshops would offer the most up to date information on how providers should communicate with ethnic minorities and the most current resources available. These workshops would also include hands on activities and scenarios similar to the ones that were provided in the Think Culturally assignment.
Douglas, Rosenkoetter, Pacquiao, Callister, Hattar-Pollara, Lauderdale, Milstead, Nardi, & Purnell (2014) outline ten guidelines for implementing culturally competent care; knowledge of cultures, education and training in culturally competent care, critical reflection, cross-cultural communication, culturally competent practice, cultural competence in health care systems and organizations, patient advocacy and empowerment, multicultural workforce, cross-cultural leadership, and evidence-based practice and research. One specific suggestion I will incorporate is to engage in critical reflection. This is mentioned both by Douglas, et al. (2014) and Trentham, et al. (2007) as an important part of cultural competency. I will do this by looking at my own culture, beliefs, and values and examining how they affect my actions. Going to a different country or area of the world can open up anybody’s eyes to see that culture makes a huge impact on the understanding and practices of healthcare that seem to be so common to other areas of the world. When a person lives in one country their whole life, that person may not realize how different the life they live is from someone in a foreign country. If a person is going to receive treatment from someone with a different cultural background, they should be expected to get treatment to respects their own culture. Massachusetts College of Pharmacy and Health Sciences having such a diverse variety of students has their own cultural competency definition that states “effectively and comfortably communicate across cultures with patients of differing backgrounds, taking into account aspects of trust in order to adopt mutually acceptable objectives and measures”. In the book Dancing Skeletons: Life and Death in West Africa by Katherine Dettwyler, the issue of culture and healthcare are greatly prevalent. Katherine Dettwyler herself goes to West Africa as an anthropologist and her horizons are broadened when during her research she comes in contact with how much culture has an impact on healthcare and everyday life.
Cultural Competence is important for many reasons. First, it can help develop culturally sensitive practices which can in turn help reduce barriers that affect treatment in health care settings. Second, it can help build understanding, which is critical in competence, in order wards knowing whom the person recognizes as a health care professional and whom they views as traditional healer, can aid the development of trust and improve the individual’s investment and participation in treatment. I want to be a nurse, specifically a nurse practitioner. I have always been fascinated with the medical field and see a bright future in it for me. I have the opportunity of meeting new people every day with a variety of personalities. There is always something new to discover in this field. A nurse practitioner's job is to be able to diagnose and treat illnesses. They also prescribe medicine and run physical exams. I am highly interested in pursuing the career of a nurse practitioner because the healthcare field is always in need for medical professionals, it pays well, and I am helping people at the same time.
Providing culturally competent care is a vital responsibility of a nurse’s role in healthcare. “Culturally competent care means conveying acceptance of the patient’s health beliefs while sharing information, encouraging self-efficiency, and strengthening the patients coping resources” (Giddens, 2013). As nurses entering the medical field understanding the culture of our patients is crucial to proper care. Each culture has their own set of beliefs and values that are shared among groups of people which influences personality, language, lifestyles, house hold, level modesty, social standings, foods, health treatment and identity. As a nurse strive to provide culturally sensitive care, they must recognize how their client's and their perceptions are similiar as well as different. To provide appropriate patient care, the nurse must understand her/his culture and that of the nurse profession. When we recognize and know a culture, we will know what is right for our patient, and thus may impose our own values on the client by assuming our values are their values. Characteristics can be as diverse as ethnic background, language spoken, gender status, physical appearance, race, and religion to name a few. Migration from various countries is creating a diverse population with different cultures and languages within the United States. Due to these cultural differences and lack of knowledge, disparities are increasing. Studies have shown that both language barriers and lack of cultural customs can hinder the services provided to the patient by the healthcare worker (Renzaho, Romios, Crock, & Sonderlund, 2013). This study provided a positive outcome when communication and cultural mutual understanding took place and patients had a more positive health outcome. It is very important that nurses are diversified in various cultures in order to better care for our patients. According to Mareno and Hart (2014), cultural competency has become one of the core values being taught in nursing programs. Their study showed that the perceived level of cultural awareness and skills among the nurses provided was low. Awareness and knowledge levels increased with higher education. The Future of Nursing Being a registered nurse affords one the option of working in many diverse healthcare settings. In any practice setting, the climate of health care change is evident. There are diverse entities involved in the implementation and recommendation of these practice changes. These are led by the Robert Wood Johnson Foundation (RWJF), the Institute of Medicine (IOM), nursing campaign for action initiatives, as well as individual state-based action coalitions. Nurses need to be prepared and cognizant of the transformations occurring in health care settings, as well as the plans that put them at the forefront of the future.
