The thesis should be clear for the reader. It should tell the reader the objective of the study. Also, the reader should have no issues comprehending the thesis. Many people do not think that its important. However, I am about to tell you otherwise. Without proper documentation, other medical staff cannot get a complete, accurate information on the patient. Proper documentation is so important because it tells about the patients’ medical history, previous surgeries, previous procedures, past diagnosis, allergies, and more. All of this documentation is important because it will help physicians with diagnosing and treatment plans. It also helps with researching. As they teach in medical courses “if it is not charted, it did not happen” . On another note documentation is used for financial reasons and of course insurance auditing. Patients medical records usually have a super bill or a physician’s dictation stating what was done and this information is used to determine how to bill the patient or the patient’s insurance company. Often insurance companies do not want to pay for certain medications or procedures so they will deny paying for it unless a physician provides a statement with medical records as to why the patient needs the medication and / or the procedure. Often insurance companies will ask for medical records to perform an audit. Audits are done yearly and if a patient changes health insurance. These days a good amount of insurance companies will not accept patients that have a preexisting condition. Why you may ask? Well depending on the condition, it means the insurance company would have to pick up all the costs from that point on plus pay for all the continuing treatment. Therefore, you could see that most insurance companies rather take the cheaper and easier way out by taking patients that do not have a preexisting condition.
