A few months after the crash, The National Transportation Safety Board is rejecting the possibility that a missile caused the blast, despite the 250 witnesses saying they saw a flare, rocket or a streak of light heading from the ocean to the plane (Jansen 4). At 13,700 feet in the air the first initial explosion happen in the center fuel tank; the nose of the airplane sheared off, the plane started to plument toward the ocean (Jansen 4). The Oklahoma City Bombing was a domestic terrorist bomb attack on the Alfred P. Murrah Murrah Federal Building in Oklahoma City in April 19, 1995. It was lead by Timothy McVeigh, an Army veteran of the Persian Gulf War. The explosive was a homemade bomb which was built by McVeigh and the help of Terry Nichols; the bomb consisted of a deadly cocktail and was put inside a rented Ryder truck in front of the Murrah Federal Building . McVeigh then proceeded out of the truck and headed towards his getaway car a few blocks away. He then started the detonation of the timed bomb at exactly 9:02 A.M. then the bomb exploded. To the people of Oklahoma it was a traumatizing moment for all, many lost families, dozens of cars were incinerated and more than 300 buildings were destroyed and caused about $652 million worth of damages. The “OKBOMB” affected hundreds of people; it killed “168 people -- 19 of them children -- and injured more than 500.” (CNN.com) Within 90 minutes of the explosion, McVeigh was pulled over 80 miles north of Oklahoma City by a state trooper who noticed McVeigh's missing license plate. He was later arrested for having a concealed weapon. From there, a investigation was held and agents found traces of chemicals on McVeigh’s clothing similar to the ones from the bomb. They learned that McVeigh’s plan was due to the anger over the events at Waco Siege two years earlier. The engine was found on Murray Street when it should have been right around the Tower. Things were said about the plane being like a missile since the engine was dropped so far from the Towers. This paper describes our nation and the worlds mindset about airline terrorism before 9/11 and airline terrorism today. This remains a very real and deadly subject even though we don’t have as many incidents occurring at this moment in time. Still the potential for countless lives being lost in an aircraft accident from the actions of a terrorist or terrorist organization is still very real and innocent families across this nation and abroad remain the targets. Additionally, it will show that the security measures in place at airports prior to 9/11 were far less adequate, than today, and personnel responsible for airport security at the gates and throughout were either poorly trained or not trained at all when it came to hijackers and terrorist. This paper will also identify the extensive security measures, rules and training that have been put into place, which helped to curtail acts of terrorism onboard airliners.
This tragic accident was preventable by not only the flight crew, but maintenance and air traffic control personnel as well. On December 29, 1972, ninety-nine of the one hundred and seventy-six people onboard lost their lives needlessly. As is the case with most accidents, this one was certainly preventable. This accident is unique because of the different people that could have prevented it from happening. The NTSB determined that “the probable cause of this accident was the failure of the flightcrew.” This is true; the flight crew did fail, however, others share the responsibility for this accident. Many things could have been done differently on the day of September 11, 2001, that could have saved many lives, including the lives of many fire fighters, NYPD officers, and thousands of civilians. Flight 93, the flight that crashed near some fields in the outskirts of Pennsylvania. But what was it doing there? What caused the plane to miss its target and crash? Many answers however can be found through other pieces of info that have come up along the way during the investigations.
The Colgan Air Flight 3407 was a very interesting case to look at. On February 12, 2009, at 10:17 pm, flight 3407 crashed at a house in New York after the pilots experience a stall. Flight 3407 was scheduled to fly from Newark, New Jersey to Buffalo, New York. The NTSB reported the cockpit voice recorder (CVR) revealed some discrepancies both pilots were experience. The first officer did not have any experience with icing condition but icing was one of the reasons the plane went into a stall. On the other hand, the captain had some experience flying in icing condition. The captain was experiencing fatigue, which indeed, made him unfit to recover from a stall. With that in mind, the Human Factor Analysis Classification System (HFACS) will give insight of some errors both pilots made.
Although the flight was scheduled to depart from JFK Airport at about 7:00 p.m., it was delayed due to a disabled piece of ground equipment and concerns about a suspected passenger mismatch with baggage. The airplane took off at 8:18 p.m., shortly at 8:25 p.m., Boston air route traffic control center (ARTCC) instructed the pilots to climb and maintain an altitude of 19,000 feet and then lower down to 15,000 feet. However, at 8:26 p.m., Boston ARTCC amended TWA flight 800's altitude clearance, advising the pilots to maintain an altitude of 13,000 feet. At 8:29 p.m., the captain stated, "Look at that crazy fuel flow indicator there on number four... see that?" One minute later Boston ARTCC advised them to climb and maintain 15,000 feet to which the pilot replied: “Climb thrust”. After an extremely loud and quick sound, the cockpit voice recorder stopped recording at 8:31 p.m. At that moment, the crew of an Eastwind Airlines Boeing 737 flying nearby reported an explosion in the sky.
