Addiction can be defined as the use of a drug or stimulus that has unreasonably taken control of a person’s behaviour. (Scollo & Winstanley 2012). It is a serious and complex issue hence the social determinants of health need to be addressed to the community and be used to guide nurses with knowledge to use in the nursing practice. Addiction to drug use has contributed to thousands of deaths, social and family disruption, violence, crime and workplace issues. In 2013, over 40% of Australians consumed alcohol, smoked tobacco or used illicit drugs at risky levels making them more likely to become addicted. The social determinants of health are social, economic and environmental factors that may influence the health of individuals. Social gradient, social support and family, early life and stress are some of the factors that can contribute to the use of a stimulus that can result in addiction. (Marmot & Wilkinson 2003) Tobacco addiction are significantly higher for people who are unemployed and those who live in socially disadvantaged areas. (Claydon et. al 2014) This population often result to tobacco and alcohol use due to poor education, financial difficulties, unemployment and no family support. Social gradient is one of the major factors that can contribute to addiction as poorer social and economic circumstances can lead to stress, thus individuals are more likely to become addicted to a stimulus, to distract them from their situation. (Marmot & Wilkinson 2003) Individuals who have lower social economic status tends to have lack of education which contributes to unemployment or insecure employment. Unemployment is a huge factor in addiction as the anxiety and hopelessness felt when individuals are unable to find a job, can result to individuals seeking relief from the stressful situation. Many people dislike the term ‘addiction’ in relation to drugs or other substances, particularly as it infers that a person is powerless over their use of a particular drug or in some circumstances, a number of substances. Whilst others maintain it is this powerlessness that is the foundation of diagnosis and treatment – that treatment is not possible without recognition of addiction itself as the ‘problem’ being addressed. The professional and public perception of addiction is complicated. There are many approaches and models to explain addiction, the role of the addict, and their environment. This essay will compare and contrast two of these approaches, the medical/disease and the social model. Initially this essay will describe the origins of each model, and follow by explaining their respective strengths and weaknesses, and finish with an overview of the key differences between them. This essay will conclude by demonstrating that a holistic approach, and a cross-pollination of these models is the most successful approach to treating addicts. The multi-causal model of drug abuse takes into account social and individual causes of addiction, both distant and immediate, that lead to a disposition to using drugs, drug use and the social and individual consequences. Socio-economic class or socio-economic status (SES) may refer to mixture of various factors such as poverty, occupation and environment. It is a way of measuring the standard and quality of life of individuals and families in society using social and economic factors that affect health and wellbeing ( Giddens and Sutton, 2013). Cockerham (2007 p75) argues: ‘Social class or socioeconomic status (SES) is the strongest predictor of health, disease causation and longevity in medical sociology.’ Research in the 1990s, (Drever and Whitehead, 1997) found out that people in higher SES are generally healthier, and live longer than those in lower SES.
Often in practice, we as nurses deal with a variety of diseases and treatments and often have to react to the illness that the patient presents with upon our interaction. While this is an essential piece of our practice, we also have a duty to our patients to be proactive in preventing specific health-related consequences based on their risk factors and to promote their health and well being. Substance abuse and addiction have become a social problem that afflicts millions of individuals and disrupts the lives of their families and friends. Just one example reveals the extent of the problem: in the United States each year, more women and men die of smoking related lung cancer than of colon, breast and prostate cancers combined (Kola & Kruszynski, 2010). In addition to the personal impact of so much illness and early death, there are dire social costs: huge expenses for medical and social services; millions of hours lost in the workplace; elevated rates of crime associated with illicit drugs; and scores of children who are damaged by their parents’ substance abuse behavior (Lee, 2010). There is a strong correlation with an individual or group SES and the quality of health care received. Social Economic Class relates to what group of class an individual fit in based on their income, which can include wages, investments or other source. The quality of care depends on the facilities that is offering the services, the staff, accessibility to the service and the kind of health insurance that the person has. Affording health care is expensive and the lower or poor class has to decide between being able to afford food or other daily needs and going to a clinic for screening. Most of the time, individuals who fall in the class will ignore the health signs while continue to work to feed the family. Individual who are in the middle class or upper class will have more access to resources such as better health insurance, access to better health facilities because they have cars and able to take a day from work, and having and higher education give you an advantage over other class. We can look at the southern belt to see how individuals with living in the southern states of the United States have deteriorating health and are more prone to diabetes, coronary heart disease, cancer and death.
