Nurses who regularly see children in their practice are currently treating victims of child abuse, including sexual abuse and neglect, whether they know it or not.
The causes of child abuse and neglect are complex and varied, and occur in all types of families and settings. Nurses must always remain alert to the possibility that abuse may be occurring in any family, regardless of their socioeconomic status.
Nationally, in 1993, averages of 42.9 children per 1,000 were reported to authorities as victims of alleged abuse or neglect. A 1995 Gallop Poll of 1,000 parents yielded the estimate that 3 million U.S. children were victims of physical abuse by their parents, or about 44 per 1,000 children (The future of children, 1998).
Child characteristics may include, the child was prematurely born, the child has disabilities, either mentally or physically, the child exhibits certain behaviors of infancy and childhood, such as persistent crying, the child is chronically ill, and the child has already been victimized. Family characteristics may include, there is other violence in the home, substances, including alcohol, are abused by the parents or caretakers, the parents of caretakers lack necessary maturity, skills or knowledge to care for the child, parental expectations are inconsistent with the child’s developmental abilities, the caretaker is socially isolated, the family is experiencing high levels of stress from events such as loss of a job, increased financial burdens, serious illness, death in the family, separation or divorce, adult members of the family have themselves been abused as children (Ohio State Medical Association, 1992).
Definitions of the major forms of maltreatment are as follows: Physical Abuse- An act of commission by a caregiver that results or is likely to result in physical harm, including death of a child. Examples of physical abuse acts include kicking, biting, shaking, stabbing, or punching of a child. Sexual Abuse- An act of commission, including intrusion or penetration, molestation with genital contact, or other forms of sexual acts in which children are used to provide sexual gratification for the perpetrator. This type of abuse also includes acts such as sexual exploitation and child pornography. Neglect- An act of omission by a parent or caretaker that involves refusal or delay in providing health care, failure to provide basic needs such as food, shelter, clothing, affection, and attention, inadequate supervision or abandonment. Emotional Abuse- An act of commission or omission that includes rejecting, isolating, terrorizing, ignoring, or corrupting a child.

When children are hurt physically, emotionally, or sexually it is known to be child abuse. Children are known to be neglected when they are not getting the proper attention needed for children. When children are neglected or abused in any way it is the responsibility of others to report such acts to the authorities as to what they have witnessed first-hand or signs of abuse or neglect seen after the fact, these people who reports such acts are known as third parties. The proper authorities to report child abuse or neglect is called Child Protected Services also known as Child Welfare. After the abuse or neglect is reported the proper authorities will then investigate to see if the abuse or neglect is legit or fraudulent (Sedlak, 2001).
Child abuse can be Physical, Sexual, Emotional, and Physiological Abuse. Child abuse is not only just about abuse but neglect as well. The difference between abuse and neglect is, that abuse is the maltreatment of someone and neglect is failure to give the proper care to a person (Jilani, 2010). Neglect has different types as well, such as Educational, Emotional, and Physical Neglect. Every place in the world has some types of statistics on Child Abuse and Neglect. Two studies performed for the National Center on Child Abuse and Neglect by Westat, Inc. provide an answer. In 1980 and then again in 1986, Westat conducted national studies of the incidence of child abuse and neglect. Each study used the same method: In a sample of counties, a sample of professionals who serve children was asked whether, during the study period, the children they had seen in their offices appeared to have been abused or neglected. Because the information these selected professionals provided could be matched against pending cases in the local child protective agency, Westat was able to estimate rates of nonreporting among the surveyed professionals. It could not, of course, estimate the level of unintentional nonreporting, since there is no way to know of the situations in which professionals did not recognize signs of possible mistreatment.
Child Abuse is an extremely significant conflict around the world; it is the physical maltreatment or sexual molestation of a child. There are 4 main types of victimization against children. Those containing physical, emotional, sexual abuse and neglect. Physical abuse is when a parent or guardian wounds their child in the form of injuring them or in any other manner causing them agony. Emotional abuse also recognized as mental abuse is when they mentally upset a child’s feelings that may generate trauma. Sexual abuse can probably be characterized as someone forcing a child in an act of unwanted sexual relationships. Neglect is the form of not providing a child with their basic physical and emotional necessities. Most people are forming organizations to prevent from future child abuse occurring but most people are also not getting involved in such dangerous crimes. Usually more than 90% of abusers tend to be people children know, love, or trust, according to Bright Futures 4 Kids.
The mistreatment of children is classified by four types of actions: physical abuse, sexual abuse, psychological abuse, and neglect. Although, in recent years several steps have been taken to prevent the maltreatment of children it seems that child abuse is still prevalent in today's society. Countless children around the world suffer from some form of abuse and in many cases the same child experiences more than one. There is no exact number of victims because it difficult to measure the exact amount of children going through abuse. As reported in Child Maltreatment 2013, out of the estimated 905,000 victims of child abuse and neglect reported in the United States in 2013, 8.8% were victims of sexual abuse. 1 This means that in that year over 79,600 children were sexually abused in the United States. “There is general agreement among mental health and child protection professionals that child sexual abuse is not uncommon and is a serious problem in the United States.” 2
There are different types of child abuse. One type is physical abuse. It involves the parent inflicting injuries to the child’s body. These injuries can include: bruises, burns, broken bones, head injuries and even injuries to internal organs (General Information about Child Abuse). As a result of physical abuse, children may be scared and shy when people approach them and try to touch them. Another sign that abused children might show can be them wearing long clothing in order to cover and hide their injuries (General Information about Child Abuse). They do this in order to prevent outside factors from asking them questions. The main sign of this kind of physical abuse that the parents or caregivers can show are anger problems or the excessive need for control.
I will be describing the first three. Emotional Abuse (also known as: Verbal abuse, mental abuse, and psychological cruelty) includes acts or the failures to act by parents or caretakers that have caused or could cause serious behavioral, cognitive, emotional or mental disorders. This can include parents and/or caretakers using extreme or bizarre forms of punishment, such as the child being confined in a closet or dark room, being tied to a chair for long periods of time, or threatening or terrorizing a young child. Less severe acts, but no less damaging, are overly negative criticism or rejecting treatment, using degrading terms to describe the child, constant victimizing or blaming the child for situations. Neglect (the failure to provide for the child?s basic needs) can be physical, educational, or emotional.

Munro (2003) states child abuse is treating a child in a way that is harmful or morally wrong however, goes on to explain it is hard to find a universal agreement of the definition of child abuse. NSPCC (2010) further explains child abuse as maltreatment of a child. This can be done directly by inflicting harm or indirectly by failing to prevent harm from happening. Abuse can be perpetrated by an adult or another child. Working Together to Safeguard Children (2013) states that it’s the responsibility of everyone who works with children to make sure that a child is safeguarded and their welfare promoted. The Purpose of this essay is firstly to discuss risk factors posed to Simone and Sasha through the scenario provided and highlight ways a professional should assess whether a child is suffering from an abusive or neglectful situation. Secondly the appropriate action that needs to be taken, and the support that needs to be given, and what services will be involved with the family in order to prevent a family brake down while safeguarding and protecting the children’s welfare, will also be discussed. Lastly this essay will draw on current legislation, guidance and procedures that promote multi– agency working in order to effectively safeguard and promote children’s welfare.

Today child neglect is the largest part of child abuse in the United States, and almost two-thirds of all reported cases in child protective services is neglect (Dubowitz). Before the 1970’s child abuse mostly referred to physical abuse; however, now it encompasses physical, emotional, and sexual abuse as well as neglect (Compton’s). Physical abuse is when a child is hurt by getting hit, punched, kicked, or any other form of beating by an adult (Compton’s). This abuse will only harm the child physically and though it is horrific and can lead to long term effects it will normally only lead to violence. This is detrimental to a child but in most cases the child will block this out for when they grow older it is like it never happened which will cause no harm to their mental state. Neglect is when an adult will not seek medical help for their child and will not provide them with food, shelter, clothing, or emotional support (Sullivan). According to the Department of Health and Human Services (DHHS) (n.d.), the causes of child abuse are 1) poverty, 2) less education, 3) unemployment, 4) changes in address, 5) marital problems, 6) lack of support, and 7) isolation. These factors are not only individual problems, but also social problems. Also, factors are considered to be interrelated to each other. For instance, parents in poverty are at higher risk of abused their children because they are more likely to struggle with economic problems because of unemployment. They are less likely to have stable jobs because of lack of education.
