One of the Model Standard incorporated in the Richmond Health Plan is the assessment of community needs (Arredondo, 2014). According to the plan, the major health problems that threaten the community have been clearly identified from data sets with information derived from the community’s perceptions of which problems are the most crucial. For instance, the plan has outlined cancer and other lifestyle diseases such as diabetes type 2, obesity and stress as major health problems that pose high risks to the residents of Richmond. In addition, the community health plan outlines the health problems faced by children with asthma. According to Richmond’s community health plan, some of the intervention strategies outlined are screening programs that will be conducted on women to identify cases of both cervical and breast cancer. Also, the health plan outlines on intervention strategies to reduce lifestyle diseases caused by poor nutrition. Due to the abundance of fast food chains and restaurants in the community, the health workers as indicated in the plan are required to educate the residents about the adverse effects of eating fast foods on a regular basis (Vojta, Koehler, Longjohn, Lever, & Caputo, 2013). It acts as a national guide for disease prevention and health promotion. There are various objectives and goals which serve as the guideline to achieve overall improvement in health . ‘Educational and Community-Based Programs’ is one of them. According to U.S Department of Health and Human Services the main goal of these programs is to “Increase the quality, availability, and effectiveness of educational and community-based programs designed to prevent disease and injury, improve health, and enhance quality of life.” (HHS, Healthy People 2020, 2011). This particular goal of Healthy People 2020 focuses on increasing the number educational and community based programs to increase awareness about disease prevention and health among every individual of the society. It focuses on educating them on various topics including chronic diseases, substance abuse, prevention of injury and violence to encourage and enhance health. It emphasizes on increasing the quantity and quality of such programs so that a larger population could benefit from the program and gain effective knowledge to achieve a healthy life.
This report has been written to explore what the context of healthy communities is. The defined community will be introduced and a geographical map included. A community can be defined as a group of people, who live, learn, work and play in an environment at a given time. (Yiu, 2012, p.213) There are many factors that may influence the community’s development and health status. These can include resources available, accessibility, transportation, safety, community needs etc… These influences may combine together to form community strengths and as well as community challenges or weaknesses. As a community health care nurse, it is significant for us to assess and identify these strengths and challenges within the community in order for us to intervene and provide the appropriate needed health care services for the community members. Today there are so many factors that affect the quality of health in our communities. In the project, I will assess the quality of health in my community. Throughout history the church has been a major factor and good starting point when attempting to understand the community. For this project I choose New Hope Baptist Church located at 284 Vine Street in Jackson, Ms pastured by Rev. Dock Cooper III. New Hope is located in area code 39206, and approximately 2 minutes from the Interstate 55, and 10 minutes from downtown Jackson. There are several churches located in this zip code (39206); also Highland Village shopping center is one of the more Up-scale shopping centers in Mississippi. This shopping center has over fifty stores, clothing jewelry, sporting and pharmacy. Schools are also spread out over this area Elementary, junior high and high schools. Jackson Academy is also located in the area, this school is widely known as one of the finest schools in Mississippi. Most of the homes and companies are old and are currently under re-construction. The population in the area is very high comprised of mostly middle-aged and young adults. The Jackson Police Department’s precinct #4 is located west of New Hope Baptist Church. Union Planters bank and Trust mark bank and a branch of the post office located in the center of the community. This community is well known as being in, “The Middle of everything”. And finally this is the community I have lived in the past 8 years.
Public Health Nursing (PHN) aims to improve the wellbeing of the population by promoting health and preventing disease among all people in the communities (Public Health Nursing, 2013). The PHN utilizes the primary, secondary, and tertiary prevention to help improve the health of the communities. The PHN process is applied to all levels of practice. Interventions are “actions take on behalf of individuals, families, systems, and communities to improve or protect health status” (Stanhope & Lancaster, 2012, p. 191). This paper will identify PHN interventions such as screening, outreach, and referral and follow-up, health teaching, and counseling that was identified in the PHN in the 21st Century project that this author completed as part of the PHN experience. This paper will also identify if the interventions were at the community, system or individual/family level.
The purpose of this community profile is to discuss a particular health improvement issue within a chosen community. A community can be defined using many different terms. You can belong to a community through religious beliefs. Through shared experiences or interests. You can belong to a political community where all involved share the same political interests. A community can also be defined as ‘a family’ a small village where many have lived most of their lives who share the same desire to belong to that community.
This is a community profile that aims to identify a specific health improvement issue within a local geographically determined community. ‘A community profile is an attempt to describe a particular community or neighbourhood. It uses a variety of different techniques to build up a picture of the community from a number of perspectives’ (Barnardos.org.uk, 2004) including several components of a community such as its demographic characteristics, patterns and trends i.e. its epidemiology in order to make comparisons between different localities to determine areas needful of specific health improvements with the goal of improving local people’s health and reducing health inequalities. A core definition of community as distinguished by Macqueen and Mclellan et al (2001) is ‘a group of people with diverse characteristics who are linked by social ties, share common perspectives, and engage in joint action in geographical locations or settings’.
Determining priorities means taking into account the trends and patterns in health data and to apply lessons learned from research. It is also crucial to include expectations from community residents, community partners and decision makers. The availability of resources, both monetarily and in manpower, to implement a possible priority must also be taken into consideration. The severity of the problem and number of people affected, disparity/equity and values are also a determining factor for establishing health priorities (APEXPH, 1991). Also, health problems that may cause death or have significant comorbidities are considered for determining community improvement priorities as is the “consequences of inaction “ (archived.naccho.org, n.d.).
There are numerous public health problems that can be addressed in my Southside of Chicago community. Among the several public health problems facing my Southside of Chicago community there are two that are more urgent. Health education or one might say lack thereof is a problem that needs to be addressed. My community is plagued with many of the residents suffering from high blood pressure, diabetes, and the killer virus known as HIV. In most cases these conditions can be prevented with healthier lifestyles and access to nutritious organic foods. In addition, environmental health is another urgent problem my community is facing. Access to clean, safe water and air is supposed to be a fundamental human right aimed at a healthy environment. Yet, my community consists a waste contaminated beach, numerous deteriorated building that are still occupied, and a countless number of restaurant and stores supplying our residents with services that are endangering their health.
An idea of a healthy community with a safe environment, accessibility to preventive health care services and screenings, education on health promotion that is stress-free and accessible and healthier grocery stores. Characteristics of a healthy community are ideas, activities, and resolutions derived from partnerships of diverse groups, regular self-evaluation that is part of the practice, which is the focus. A diversity of strategies, techniques, documents, and other resources would have to be executed to support and make a healthy community. If an individual 's knowledge of a healthy community is different from his or her belief then community strategy must be put in place in order to begin to achieve the goal of improving health. Community strategy must be supported by the individuals who are working together in order to be successful (Matthew, et al, 2001). In order for a strategy to become effective, a consistent plan should be created with achievable goals within a community.
