Mrs. Suzy Bae slipped and fell on the way to the washrooms, and sustained an injury to her scalp. On Monday, September 28th, 2015 at approximately 2:01 pm, I had received a call from room number L205 by Mrs. Suzy Bae to for an escort to the washrooms. Reached the room at approximately 2:12 pm. Helped Suzy out of bed, and made the way down to the washrooms. On the way to the destination, at 2:15 in hallway LC, Suzy suddenly lost her balance and slipped on a recently mopped floor. It was due to there not being a wet floor sign in sight, and not seeing that the floor was recently mopped. As she fell backwards, in consequence, she hit her head on the edge of a nearby table.
