On Tuesday, May 17, 2016 at approximately 1956 hours, Officer Reinbold #7400 and I (Officer Guerrero #3310) were dispatched to 1113 Marengo Avenue, in regard to a vandalism that just occurred. Dispatch advised the victim (later identified as Kristyn Cota) saw two male juveniles spray painting her fence. Cota advised dispatch she was following both subjects and they were last seen running southbound on Marengo Avenue, from Hammond Street. The first suspect was described as male Hispanic, approximately 15 years old, wearing a gray shirt and dark colored shorts. The second suspect
Chloe Valentine lived in Ingle Farm with her mother Ashlee Polkinghorne, 23 and her mother’s partner Benjamin McPartland, 28 . Together they thought it was a good joke to force Chloe to drive a motorbike three times her weight . The motorbike was a 50cc dirt bike of 50kg while Chloe only weighed 17kg . Chloe’s estimated speed on the motorbike while driving was 40km/hr . Rosa Lee Cunningham is a 56 year old African American female. She is referred to the facility from Howard University Hospital. She was treated for a condition called osteomyelitis, which resulted from a bacterial infection while using heroin (Dash, 2006). Rosa Lee states that on October 7, 1983, she injected cocaine, which resulted in her being hospitalized at D.C. General Hospital (Dash, 2006). Prior to her hospital visit in 1983, she injected heroin, cocaine, and various substances. After a horrible breakup with her girlfriend, she used heroin for the first time (Dash, 2006). She stated that she uses speed ball of cocaine, heroin mixed injection as well as Prelundin, occasionally (Dash, 2006).

Ashley Smith was a young girl that was placed in a juvenile detention centre at age 15 for throwing apples at a mail man. Her short sentence quickly extended into a life sentence because of so many infractions within the prison system. Ashley suffered from extreme mental health issues and was place in a psychiatric prison facility, however this facility was shown in the documentary to be corrupt and their actions with Ashley were extremely illegal. Furthermore, Ashley wasn’t given the proper help and treatment that she needed, instead she was physically and verbally abused by guards in the prison, and she ultimately passed away in the prison. On 2/8/18 at approximately 1845 hrs a fight was called over the radio, Sgt Barrett, Kyle responded. When Sgt Barrett arrived at D Bldg, Sgt Barrett saw I/M Hills, William (GDC 524378 W/M STG: Aryan Brotherhood) outside next to the D1 window bleeding from his head. Sgt Barrett then went to D1 cell 24 to check on I/M Swann, Stacy (GDC 820663 W/M No STG) was his room extremely agitated. While Sgt Barrett was talking to I/M Swann Sgt Barrett noticed that there were lacerations to his head, blood on the floor and on his hands. Sgt Barrett then instructed I/M Swann to come to the door to be handcuffed, I/M Swann complied. No force was used. I/M Hills was escorted by Sgt James from D yard to Medical for evaluation. After I/M Hills left the yard Sgt Barrett then escorted I/M Swann from D Bldg to Medical for evaluation.
On Thursday, August 3,2017 at about 0900 hours, I was present at Kings County Criminal Court, Part AP1F for SPAA Tammie Croswell's scheduled court appearance. SPAA Croswell pled guilty to a violation (P.L 240.20). She was ordered to make restitution in the amount of $4,200.00 plus a surcharge of 5% ($210.00). In my opinion, David Cash should be punished for either not reporting to the police or stop Jeremy for sexual assault or murdering Sherrice, because David witnessed Sherrice being molested by his best friend and did nothing about it, he just walked away. Then Jeremy came to David and confessed to him that not only he molested her, he murdered Sherrice less than half an hour ago and David didn’t ask him why Jeremy did it? And since David failed to report to the police when Jeremy informed him about his crime, he should be charged and prosecuted as an accessory for both sexual assault and murder of Sherrice Iverson.
