Reporting party (RP) received a call from a "whistle blower", Regina Horn, who stated that she is a former employee of this facility. Ms. Horn reported that the administrator has been verbally and physically abusing the 3 residents that reside in the home: Brandy Venn (DOB: 08/02/81), Kristal Murphy (DOB: 05/03/95) and Ashton Murphy (07/31/93). Ms. Horn stated that the administrator yells at the residents and even uses a belt to hit them. Ms. Horn stated that she worked at the facility for 3 months until 07/19/17 because she could not take it anymore. It was reported by Ms. Horn to RP that she is really not employed at the home since she was only getting paid cash. RP asked Ms. Horn why she was not hired as an employee and Ms. Horn mentioned
that it had something to do with her background check. Ms. Horn also mentioned that staff Charlotte was hired despite of an allegation of her abusing a resident at another care home. Ms. Horn disclosed that the administrator has been diagnosed with Alzheimer's and is facing divorce at this time. RP visited the facility and spoke with staff and discovered that there were 2 new staff members working there; Flory Rutherford who has been employed for 1 1/2 months and Tina Mikaio for 3 weeks. When RP asked them the names of the other employees, they did not mention Ms. Horn's name and claimed that there was no one working at the facility by that name.
As the EAI team was discussing Molly’s case, one of the ED Residents made a few telephone calls. Molly’s PCP reported that during her last visit about 2 weeks ago, Mollie was alert and able to respond to questions appropriately. He confirmed that Mollie’s daughter and son in law have experienced psychiatric problems, adding that the son in law has expressed anger regarding Mollie’s living arrangements. A mandated report has a specific way that it has to be field out because the severity of the information is a massive part in helping save an abused or neglected child from being endangered. This research analyzes the difference between the different documentations in mandating reporting and how one reports various from another. The next step is to investigate where and who the forms are turned into. I’ll be looking at the Social Worker perspective because it’s the field of study I’m going in and would potentially help me when I begin my career in children and family services. Giving the information about mandated reports would educate me on the all the factual information needed after receiving a mandate report and from the information gathered, how do they deiced whether or not it’s a serious case or not. This research would uncover every aspect from beginning to end of the entire process of when a report is submitted up until the discussion is made on what to do after reviewing the information. The report is used my any profession to report suspect child abuse or neglect at any time or place. There is a different between different careers on how the report is written depending on the person submitting it. The very last step is going in depth with analyzing the actual form and comparing and contrasting it to other forms from different states. I want to also look at, the different between the forms, depending on who is the attended audience.
Reporting party (RP) is a medical assistant for Dr. Seema Sangwan's office who is the PCP for resident Steven Staub (age 62, DOB: 09/06/54). Resident is a paraplegic and dysphagia. Dr. Sangwan spoke with the resident privately and resident disclosed that the caregiver was in love with him and wanted him to move in with her. On 6/23/16 the client Cristina Maldonado #130 did not show up or contact her assigned worker for the scheduled ILP meeting .Case Manager went to unit #130 and meets with Ms. Maldonado and inquired about her noncompliance her weekly meeting; she stated that she has no reason to come for her meeting because she don’t think anyone is helping her here. Upon arrival on the scene we were met by officer Hamrick of the Metropolitan Police Department, 4th District.
