There have been many changes (major and minor) in the nursing profession and the medical field, as a whole, over the years. Some of these changes include changing demographics and increasing diversity, technological explosion, the cost of health care, and significant advances in nursing science and research. Major socioeconomic changes created this drive to make transformations in nursing and institutions. These institutions are the ones that are responsible for educating the next generation of nurses, such as West Coast University.
Over the years, there have been major shifts within the United States in regards to its population. Statisticians have estimated that 20% of the population will be 65 years of age and older. There is an even larger number of the population that will be over the age of 85. Unfortunately, variations in morbidity and access to health care have been a cause for increasing violence and substance abuse (Barbara R. Heller, Marla T. Oros, and Jane Durney-Crowley, 2013). This article states that due to these new disparities, “Nursing practice, education, and research must embrace and respond to these changing demographics, and nurses must focus on spiritual health, as well as the physical and psychosocial health of the population (Barbara R. Heller, Marla T. Oros, and Jane Durney-Crowley, 2013, para. 2).” Nursing schools now have a variety among there students, which has increased drastically. This increase in diversity has, “created a rich cultural environment for learning (Barbara R. Heller, Marla T. Oros, and Jane Durney-Crowley, 2013, para. 3).”

Advances in processing capacity and speed have contributed to the explosion of information technology applications. One issue still remains, that is the lack of health care coverage for many Americans today. It is a saddening and stressful moment for health care providers, especially nurses, across the globe. Hopefully, Obama Care will do more for the population than past health care reforms. Furthermore, we must remember that research in the nursing field is done in order to improve the outcomes of patients and to advance the health and well being of communities. Everything must be done in order to create more funding and create more opportunities for more nurses, in order to help the ever-growing population.
The IOM report had four key messages needed for advancing the future of nursing. “Nurses should practice to the full extent of their education and training; achieve higher levels of education and training through an improved education system that promotes seamless academic progression; be full partners, with physicians and other health care professionals, …and; effective workforce planning and policy making require better data collection and an improved information infrastructure” (Institute of Medicine, 2011). The report also included eight recommendations needed to facilitate the necessary changes to in the nursing profession so meet to demands of the healthcare reform.

The article I chose discusses the continual change in the roles of nurses. The article also poses a concept that nursing now is not based on caring, but medicine. “By accepting continual changes to the role of the nurse, the core function of nursing has become obscured and, despite assuming medical tasks, the occupation continues to be seen in terms of a role that is subordinate to and dependent on medicine.” (Iley 2004) Nurses are taking a more professional role, and more tasks are being delegated to assertive personnel. Therefore, with all these changes occurring, the role of the enrolled nurse is unclear. “Previously, having two levels of qualified nurse in the United Kingdom had been seen as problematic for health service managers and nurses themselves, and the ending of enrolled nurse programs in 1992 helped to solve this problem.” (2004) The study in this article gathered the characteristics of enrolled nurses and differentiated the groups converting to registered nurses, groups in the process of conversion, and groups interested or not interested in conversion. This study reveals the situation of enrolled nurses in context of continuing towards the professionalization of nursing. “The data from this study support the possibility that the role of nurses as direct caregivers is seen as a positive dimension of the work they undertake.” (2004) The findings imply that nurses need to get back to being caregivers, instead of concentrating on obtaining professional status in medicine.
There is a shortage of all health care professions throughout the United States. One shortage in particular that society should be very concerned about is the shortage of Registered Nurses. Registered Nurses make up the single largest healthcare profession in the United States. A registered nurse is a vital healthcare professional that has earned a two or four year degree and has the upper-most responsibility in providing direct patient care and staff management in a hospital or other treatment facilities (Registered Nurse (RN) Degree and Career Overview., 2009). This shortage issue is imperative because RN's affect everyone sometime in their lifetime. Nurses serve groups, families and individuals to foster health and prevent disease.
Nursing research is a systematic enquiry that seeks to add new nursing knowledge to benefit patients, families and communities. It encompasses all aspects of health that are of interest to nursing, including promotion of health, prevention of illness, care of people of all ages during illness and recovery (or) towards a peaceful and dignified death (ICN 2009)
Current literature continues to reiterate the indicators of a major shortage of registered nurses (RNs) in the United States. The total RN population has been increasing since 1980, which means that we have more RNs in this country than ever before (Nursing Shortage). Even though the RN population is increasing, it is growing at a much slower rate then when compared to the rate of growth of the U.S. population (Nursing Shortage). We are seeing less skilled nurses “at a time of an increasingly aging population with complex care needs and an increasingly complex technological care environment” (Mion). According to recent data from the Bureau of Labor Statistics and the Department of Health and Human Services, it is estimated that “more than a million new and replacement nurses will be needed over the next decade” (Diagnosis: Critical).

It is too late to take nursing back to the point of basic care of the 19th century now fulfilled by certified nurse aides. A nurse is the first and last person many people will ever see. They provide complex care, recognize symptoms and changes when the ever shrinking physician pool is not available, advocate for patient and families, and care deeply for their patients recovery. In this paper the author used past nursing history, current medical demands and advancements, and clinical patient outcomes to argue that the minimum education for a nurse should be a Baccalaureate degree in order to meet the Institute of Medicines goal of 80% of nurses should have BSNs by 2020.
Nursing has always been a key career in the health care system. Although it is not often focused on in media and stories surrounding health care, nursing is a career of great importance. If any patient was asked about their experience at a hospital or a care center, many will mention the capability and care that they received from the nurses. The health care system could not function efficiently, if at all, if nurses were not present to perform their part. Nurses are more than just physicians support staff. Of interest in this paper is why people choose to study nursing in university or college. From students just graduating high school to mature student who have prior degrees or education, nursing is a popular choice as a path to study
Since the 1990’s, the interest in nursing and the profession as a whole has decreased dramatically and is still expected to do so over the next 10-15 years according to some researchers. With this nursing shortage, many factors are affected. Organizations have to face challenges of low staffing, higher costs for resources, recruiting and reserving of registered nurses, among liability issues as well. Some of the main issues arising from this nurse shortage are the impact of quality and continuity of care, organizational costs, the effect it has on nursing staff, and etc. However, this not only affects an organization and community, but affects the nurses the same. Nurses are becoming overwhelmed and are questioning the quality of care that each patient deserves. This shortage is not an issue that is to be taken lightly. The repercussions that are faced by both nurses and the organization are critical. Therefore, state funding should be implemented to private hospitals in order to resolve the shortage of nurses. State funds will therefore, relieve the overwhelming burdens on the staff, provide a safe and stress free environment for the patient, and allow appropriate funds needed to keep the facility and organization operational.
In 1965, the American Nurses Association shared its vision for the future of nursing education. A primary goal in its message was that a Bachelor of Science in Nursing (BSN) becomes the minimum requirement for entry into nursing practice (Nelson, 2002). Fifty years later, debates on this issue continue. In its report, The Future of Nursing: Leading Change and Advancing Healthcare, the Institute of Medicine shared its recommendation that 80% of nurses possess a bachelor’s degree by the year 2020. A significant number of health care facilities have adopted the practice of hiring nurses with BSN over those with two year Associate’s Degree of Nursing (ADN) or a three-year hospital based diploma program. For nursing, research has played a major role in the way clinical practice is done. Research has allowed nurses to provide appropriate care to patients. Throughout time, nurses have worked hard, whether the profession was well respected or not, which is how many of the different changes have happened in nursing. This profession has been evolving continuously to shape what is happening in the nation and the world to continue to help the sick and injured. You can see throughout time what influences the events had on the changes in the field of nursing. Nursing will continue to evolve with new and different hospital structures and more advanced technology. No matter how advanced the gadgets become or how complex the job is, nurses will remain being caregivers and with vouch for the sick and at risk members of our communities.
There has been a drastic cut in both the Medicare and Medicaid reimbursement rates. According to Robert (2012), “cuts of more than $360 billion to Medicare and Medicaid will be made over the next 10 years. The focus of nursing needs to emphasize more on wellness care and prevention rather than acute care” (McNeal, G., 2012). Nursing practice will need to shift more towards community and population focused nursing. Baccalaureate prepared nurses are provided with a curriculum that includes both community health and leadership skills that are not included in the associate program. By encouraging associate’s degree nurses to obtain a bachelor’s degree in nursing, an increase in the awareness of the needs of the community and population may be seen. The IOM report has outlined the anticipated obstacles that healthcare will face if changes are not made. By allowing nurses to provide care within their full scope of practice, quality care may be provided at an affordable cost to the population. The use of advanced practice nurses in primary care may provide quality, access, and cost efficient healthcare to high-risk populations and possibly decrease hospital admission rates, thus lowering the overall cost of healthcare. The Future of Nursing Being a registered nurse affords one the option of working in many diverse healthcare settings. In any practice setting, the climate of health care change is evident. There are diverse entities involved in the implementation and recommendation of these practice changes. These are led by the Robert Wood Johnson Foundation (RWJF), the Institute of Medicine (IOM), nursing campaign for action initiatives, as well as individual state-based action coalitions. Nurses need to be prepared and cognizant of the transformations occurring in health care settings, as well as the plans that put them at the forefront of the future.
