Although, project 4031 fulfills wishes for terminally ill clients, they are not always informed when a client has passed away. Therefore, we had to revised knowledge goal 1 to instead prepare Project 4031 for future bereavement phone calls. Knowledge Goal 1 was revised once I was nearing the end of my internship at Project 4031. Instead of learning the communication strategies I had hoped for, what I did instead was write up a document for Project 4031 to have and look at when they are making their bereavement phone calls. The document is a form to have for future volunteers or the staff to use while they make the bereavement phone calls. This practicum project experience project plan is the initial step in addressing an identified deficiency in the new CPOE/clinical documentation system implemented last year at the medical center at which I am employed. Using the standardized tools of project management, I have presented the project objectives, the global project methodology, and the formative and summative evaluation criteria. To further describe this project plan, a graphical timeline is shown in figure 1. A narrative explanation of the timeline is included in the appendix. Finally, this project plan is supported by scholarly and peer-reviewed literature.

The document was chosen because I have a particular interest in palliative care and those suffering from chronic pain. Pain effects the quality of life effecting large numbers of the population, especially those near the end of life.
Breavement is handeled differently in different generations. Weather it is a kid that has a terminal illness or an elderly person who is diagnosed with a terminal illness, each breave differently. Breavement deals with not just someone clsoe dying but, someone themselvs who is diagnosed with a life threatening illness.
I was shocked by the manner in which the professionals spoke about death and the dying, as a business transaction. This meeting was a blatant reminder that the business side always has a presence in some degree. As a social worker, I felt uncomfortable with the way they were speaking. It did not seem appropriate for the social workers to stop everything and give a lesson on the dignity and worth of a person. The best way to combat this may be to change the way we speak about patients amongst ourselves. Focusing on one person at a time, the sensitivity and awareness can grow into the fabric of the office culture. Study Purpose The purpose of this research study was to develop a communication strategy for family members of patients dying in the ICU by evaluating a format consisting of a proactive end-of-life conference and brochure to see whether the intervention could minimize the effects of bereavement for the families left behind. Your time is on earth is limited one day you will die and there are many ways of grieving at the death of a loved one. I believe that the fear of death and the death of a loved one will hold you back from living your own life and the fear of your own death is selfish.
