Next, place the jack just to the front of the flat tire and pump or crank it up securing it to the frame of the car. Jack up the vehicle until the flat is at least six inches off the ground, then unscrew and remove the lug nuts completely. Now remove the flat tire from the hub by grabbing it securely with both hands and pulling it off. Remove the spare tire from the vehicle, and install it onto the hub by mounting it on to the lug bolts and pushing it until the lug bolts are through the rim, then put the lug nuts back on to the lug bolts and tighten them all by hand. Do not use the lug wrench to tighten the bolts until the vehicle is lowered back down; it can cause the wheel to simply rotate, or the car may fall off the jack.
