According to the U.S. Department of Justice, African Americans are more likely to be incarcerated compared to any other major ethnic groups (2010). One of the crucial problems or challenges experienced within prison is the need to provide appropriate mental health treatment services for the applicable diagnosis (U.S. Department of Justice 2011). However, these diagnoses are often skewed or directly influenced by race or an inmate’s racial background forming stigmas toward seeking out treatment among the other inmates. Stigma and race correlated with mental health diagnosis in a penitentiary or correctional facility continues to be a prevalent or widespread obstacle that leads to negative attitudes about mental treatment and ultimately deterring individuals who need services from seeking medical or psychiatric care.

As of now, there is no general consensus that would require states to cover mental health (Cauchi, Landess, & Thangasamy 2011). Out of the 49 states that do cover mental illness, there are three main categories that vary considerably; mental health “parity” or equal coverage laws, minimum mandated mental health benefit laws, and mental health “mandated offering laws.” Mental illness is as serious a condition as any other health condition. It should be covered as such. Mental health parity has come a long way over the years. With health care reform underway, this issue is bound to come up. Hopefully the reform can enact a program that will lead to more mental health parity.
The PICO question for this research topic is, in psychiatric patients boarding in the emergency room how does having a therapeutic environment affect the amount of times that restraints are used. This topic of if having a therapeutic environment for psychiatric patients that are boarding in the emergency room decreases the use of restraints is vital in nursing and in the care of the patients. The fewer times restraints need to be used the better the patient’s experience is in the emergency room and the less likely the staff members could get hurt. Using physical and chemical restraints are of last resort. These two interventions can be traumatic for the patient and the staff members. That being said, if implemented a therapeutic environment for the patient will decrease the use of these interventions, it is something that
